Senior Application for Graduation aka “Senior Letter”

How do you want your name on your diploma How do you want your name on your diploma? Make sure it’s some version of your legal name. Please make any necessary corrections in this section. Please pay special attention to Student SS#

This is how many units/credits you still need to earn to graduate This is how many units/credits you still need to earn to graduate. You can only earn a maximum of 6.0 credits every school year. If this number is more than 6.0, you need to make up credits outside of DHS in order to graduate in May! This is where you will find all of the credits you have already earned and the credits you need to earn in order to graduate. The titles of all of your classes may be listed or just some—different counselors may fill out this form differently. The important thing to pay attention to is the next point…
