Wiltshire Dementia Services Background The focus on dementia has been increasing in recent years, both at a national and local level. It is now considered as a priority area for action, largely due to the increasing population with dementia, the cost of this to services, communities and families and the variable quality of care that many people with dementia receive from health and care services. The Wiltshire Joint Strategic Assessment supports this; identifying that the number of people with dementia will nearly double by 2030 in Wiltshire. Health and Wellbeing Strategy acknowledges the increasing population living with dementia and identifies it as an area for action.

Wiltshire Dementia Services National Living well with dementia: a National Dementia Strategy (Department of Health, 2009) The Prime Minister’s Challenge on Dementia: Delivering major improvements in dementia care and research by 2015 (Department of Health, 2012). These place a focus on improving health and care services and dementia friendly communities, as well as improving people’s awareness and understanding of dementia, the importance of early diagnosis, ongoing support and the role of services in ensuring that people can live well with dementia.

Wiltshire Dementia Services Currently in Wiltshire: August 2016 figures (NHSE) 4296 – People >65yrs. With diagnosis of dementia. 6624 – Denominator (likely number of people in Wiltshire who will have dementia) >65yrs. only. Approx. 300 people <65yrs with dementia.

Wiltshire Dementia Services Historically Wiltshire Council has commissioned various dementia services since 2009. An Alzheimer’s Alliance was formed in 2011 between two specialist dementia providers. This contract brought together a number of separately funded services across the county under one contract. Dementia Services in Wiltshire have not been through a tender process before. The current contract is now due to expire on 31 March 2017 and requires a tender process is to be undertaken.

Wiltshire Dementia Services The Wiltshire Dementia Strategy Published in 2014; it was developed by Wiltshire Council and NHS Wiltshire Clinical Commissioning Group (CCG) in conjunction with various local partners from the statutory and voluntary sector, as well as through talking to people with dementia and their carers and families about their experiences in Wiltshire. The Wiltshire Dementia Delivery Board has overseen the strategy in all its pre-published development and engagement processes and has since been responsible for the implementation of the Dementia Strategy Action Plan.

This is Wiltshire….. Some useful data Wiltshire is a large predominantly rural and generally prosperous County with a population of 480,000. Almost half of the population resides in towns and villages with less than 5,000 people and a quarter live in villages of fewer than 1,000 people. Approximately 90% of the County is classified as rural The relationship between the larger towns in Wiltshire and the rest of the County has a significant effect on transport, employment, travel to work issues, and economic needs.

Cont’d…………… Wiltshire has a lower population density than the South West or England overall. The rural nature of the County has implications for the commissioning and provision of well planned services particularly with a shift towards more provision of services within the community Therefore whilst our population is relatively healthy, we recognise our future strategies will need to incorporate developing more partnership approaches for evolving services and improving outcomes

Wiltshire Dementia Services Principles: The main purpose is to ensure that people with dementia, their carers and families are able to live well and are supported to do so through being able to access the right services and support at the right time, whether that be from organisations or their local community.

Principles - Workforce Specialist dementia trained staff. Committed to the principals of the Wiltshire Dementia Strategy and it’s outcomes. Motivated to deliver outcomes – achievement. Supported by colleagues. Supported through supervision. Well paid. Career pathway.

Community Engagement. The things that people with dementia and their carers and families said are important to them and are going well include: Community activities such as memory cafes and singing and music groups. Support from the voluntary sector. Being able to meet and socialise with other people living with dementia. Day care. Telecare.

The things that people told us are important to them and need improving include: Support for people living alone. Support to plan for the future. Direct payments. Transport. Support for carers.

Understanding of professionals of the challenges of living with dementia. Processes, paperwork and the language used. More time. The general public’s understanding of dementia. Support from businesses. Person centred care .

There were some things that people with dementia and their carers and families told us are important to them, but people have had mixed experiences: Support from GPs. Specialist mental health services. Knowing where to go for information and help. Carer involvement. Acute hospitals. Care in care homes.
