Patient access – Ordering a prescription How to order a prescription using patient access

Booking appointments for patient access Click on the right hand side and log into Patient access. You will need the correct User ID and password to do this.

Patient access – Ordering a prescription Next go to your repeat prescriptions. You can do this by going down the page to “Repeat prescriptions” and clicking “request repeat prescription”

Patient access – Ordering a prescription Once you have clicked on “Request repeat prescription” you will see a list of all the medication that is currently on repeat prescription. Please note The below is just for example.

Patient access – Ordering a prescription If you have handed in a completed consent form and have received your registration document and can still see the repeat prescription option showing “contact your practice for access” please contact the surgery. If you can not see “request repeat prescription” or details of our repeat items. Please contact the surgery ** Please note a consent form can take up to 4 weeks to process. **

Patient access – Ordering a prescription To request an Item click on the box on the right hand side. If you wish to untick an item click the box again

Patient access – Ordering a prescription Only repeat items will be shown, any items prescribed to you that are not on repeat will need to be requested by entering the medications name into the message box. You must request at least one repeat in order for this “comment” to be seen by the surgery

Patient access – Ordering a prescription If you wish to make specific requests regarding your prescription (e.g to state you have lost tablets and require them early or to request more tablets for going on holiday) these requests must also be in the comments box when you are requesting a repeat item.

Patient access – Ordering a prescription If you have no repeat items then you will need to put a written request for the medication to one of the surgeries. This can be hand delivered, posted or faxed to the surgery. You will need to include your name, date of birth and a contact telephone number as well as details on the medication you require, giving as much detail as you have available.

Patient access – Ordering a prescription If you are unsure if you have received your registration document details please contact the surgery. Please be aware that a signed patient access consent form is required and this can take 4 working weeks to process. The E-mail will be titled and the letter will be addressed to the patient seeking to register. It will contain details such as linkage key and user ID

Patient access – Ordering a prescription If you need to sign a consent form please ensure you come into one of the surgeries and bring with you some valid photo ID. If you do not have any, please contact the surgery before coming in. If you believe you have already signed a consent form but still don’t have access to all the services please check with the surgery before coming in.
