Transitioning Specialist – Duties, Current Staff, and Coverage   Office of Institutional Educational Programs

Transitioning Students Back to School of Origin Key Functions of Institutional Programs Staff During Transition Provide the receiving school counselor comprehensive academic and behavioral records. Communicate with home schools & families about CIEL, educate them on what it is, and ensure they are doing what is needed for student continuation and completion of CTE credits. Participate in host agency treatment team meetings for our students' continuity of care. Participate in Multi-Disciplinary Treatment team meetings. Work with host agencies and other parties to access necessary aftercare and supportive mentoring for the transitioning students. LINK, REFER, and ADVOCATE for all OIEP students.
