Visit www.qocenter.com Click on the Upload Your Order tab on the main page Alternatively, you can select the Upload Your Order link under the Take Action section on the right side of the page
Choose Employer Select Click to Upload Your Order link
Enter Employer Name Type in the employer name in the Employer Name field Select Continue
Create One Time Password Enter the email address for the sender The recipient at the email address that is entered will receive a password that will be available for 15 minutes
Enter validation password Enter the emailed password on this screen, then select Validate OTP This process will only need to be done once for any user
Upload the attachment screen Select the type of notice that is being uploaded Enter the employee’s Social Security number or Employee ID number (this is optional) Select the Browse button to select the file to upload Select Attach Select the Captcha “I’m not a robot” icon Once Captcha has been completed, select Upload Order
Confirmation Page Once the file has been attached and uploaded, you will receive the confirmation screen
Tips for uploading Verify the PDF file is not password protected Total number of pages in file should not exceed 50 pages Verify file is 5 MB or less Limit the total size for file to 10 MB or less Send order by fax or mail if the files exceed the size limits The file formats this site accepts for document uploads are .jpg; .jpeg; .pdf; .gif; .tif; .tiff; .png; or .bmp File name must only consist of numbers 0-9, alphabets a-z\A-Z and underscore Only upload one file per employee during one session
