Immunization Exemptions TME: Temporary Medical Exemptions are for students who are in the process of completing their age appropriate immunizations. Remember there is a two year lock of TME’s! That means that the time given to update a student’s immunization status may not exceed 2 years. Permanent Medical Exemption: Part C of the 680 Florida Immunization certificate. Communicable Disease Surveillance and Reporting Procedures Common Childhood Diseases found in the M-DCPS Facilitator Manual Must Report Vaccine related illness outbreak such as mumps, measles, and pertusis ( need medical provider documentation ) Must Report illnesses such as Tuberculosis, and Bacterial Meningitis Outbreak / Contact Investigation for 2 or more of the same illness at the same school site. Integration with office of Disease Control at the Dept. of Health Briefing # 19571 gives exact instructions and the template for reporting disease outbreaks.

Measles Molluscum

When a parent/guardian reports an illness it is important to get supporting medical provider documentation to confirm the illness. If the parent / guardian reports that the student is ill in the hospital please get the hospital name, day of admission and last day of school attendance. This information must be placed on a communicable disease template scanned and e-mailed to wilmasteiner@dadeschools.net and Bwilder@dadeschools.net along with the medical provider document if there is one. Chronic Disease Management Includes parents/guardians filling out specific action plans and medication authorizations for metabolic conditions including: When the documentation has been returned to the your school site please e-mail your request for a school site training to Bwilder@dadeschools.net on the specific condition, (Do not send the documentation). Please include a contact person and contact number so that the school nurse from the health dept. can follow up. Health Suite / Clinic Log The Miami-Dade County Health Dept. has requested that a record be kept of all students who visit the school clinic. The clinic log should be shared with the Miami-Dade County Health Department’s school nurse when she visits the school.
