What is CastleBranch? CastleBranch (CB) manages our Health Records for Students Enables us to provide health records certification to your clinical locations Long term storage of all of your up-to-date immunizations and other health information for future employment CB tracks your records to let you know via email when updates are needed. Sign on to CB Go to www.castlebranch.com, click on Place Order tab on the upper right. In the place order field, enter your unique class package code which will be given to you at the time you should place your order. While placing your order you will need to know your SPIRE Number (Personal Identification Number.) Your email address that you use when placing your order will be your username for your CB account. Download the application “MY CB” on your cell phone or handheld device to have access to your records at all times. Upload documents, update content and view your status. What to do when on CB Your to-do-list is the area that informs you what is due, the exact requirement needed, when it is due and rejections. Click on each item on your to-do-list to get further information and options to upload your documents. If an item is rejected you will be notified via email and will be put under your rejection tab. Requirements are specific! TB 2 step skin test (two separate 1 step tests done 1-3 weeks apart) TB – Negative T-Spot or QuantiFERON Gold Blood Test within the last 12 months. See to-do-list for other ways to satisfy TB requirements. Physical Exam CB just needs a doctor’s note stating your name, had a physical, the date of exam, doctor’s signature and your date of admission. Must be within 12 months of your admission date. Professional Liability Insurance CPR Certification Must be the American Heart Association Healthcare Provider BLS or ACLS. American Red Cross will not be accepted. Copy of both the front and the back of your card is required and the card must be signed.  Professional Liability Insurance Submit the certificate that shows start and end date and the breakdown of your coverage. CORI There are various CORI forms you will need to access depending on the year/clinical site. CB CORI Form must be the form on the blue link of your CORI requirement. Please add the state in which your driver’s license is issued next the driver’s license number. Other CORI Forms: This CORI form is for the school that you will be going to for Pediatric or Community practicum. This will be required to be completed 3 months prior to the start of your semester. Fingerprinting You will be notified by email 3 months prior to starting clinical to make your fingerprinting appointments. FBI Check Required for specialties in public school systems IdentoGO is the company chosen by Federal Government Massachusetts has a number of collection sites around the state please go to ma.ibfingerprint.com. Fingerprinting When you book your appointment, you will need to select: Agency/Sector: “Pre-K-12th Grade Education (ESE)” Applicant Type: “All Other School Personnel” and submit the provider id numbers: These are the local public school districts. These numbers will be identified when you are triggered to do this requirement. Once completed, you will need to upload your fingerprinting receipt to CB. More Information If you do not submit all codes and you are placed in a district for which you have not had your fingerprints submitted, you will need to pay an additional $35.00 fee and have your fingerprints taken again. Drug Screening Required to enter your clinical sites during practicum. You will be notified when you must start this requirement and a due date to have it completed. Screening is ordered through CastleBranch with product code which will be given to you at the appropriate time. We will send you a reminder via email when it is time to begin your drug screening orders. Testing outside the “window” will need to be repeated. So please wait for email to instruct you on how to order and when. There is a cost of $40 for each drug screening per semester.
