The American Medical Group Association (AMGA) is a trade association representing medical groups, health systems, and other organized systems of care. AMGA’s mission is to support its members in enhancing population health and care for patients through integrated systems of care.

AMGF is AMGA’s 501 (c)(3) affiliate and it supports and disseminates research on better ways to deliver health care that is scientifically sound and value-based. AMGF also develops and supports educational programs to enable all stakeholders-providers, payers, policymakers and patients to translate the evidence of what works best into everyday practice.

RA Collaborative Overview A shared learning environment A research project designed to facilitate the discovery, adoption and spread of care processes that will improve the way in which patients with rheumatoid arthritis experience the health care system. A vehicle to leverage key learnings and disseminate best practices to all AMGA members.

How Does the Collaborative Work? Bi-monthly webinars Online resources including a dedicated website for participant material-sharing and a listserv for idea-sharing and collaboration Consultation with experts in the field Two in-person meetings At the end of the 12-month collaborative, AMGF will work with participating organizations to develop individual Best Practices Case Studies of their programs for publication

Bi-Monthly Webinars : 2nd Thursday of every other month Thursday, October 9, 2014, 2:00-3:30pm (EST) Thursday, December 11 2014, 2:00-3:30pm (EST) Thursday, February 12, 2015, 2:00-3:30pm (EST) Thursday, April 9, 2015, 2:00-3:30pm (EST) Thursday, June 11 2015, 2:00-3:30pm (EST) Wrap-up Meeting: August 2015, Location and Date TBD

Measurement and Reporting 1. RA: Disease-Modifying Anti-Rheumatic Drug (DMARD) Therapy (NQF 0054, PQRS #108) Percentage of patients aged 18 years and older diagnosed with RA who were prescribed, dispensed, or administered at least one ambulatory prescription for a DMARD, or who had a DMARD on their active med list 2. RA: Periodic Assessment of Disease Activity (PQRS #177) Percentage of patients aged 18 years and older with a diagnosis of RA who have an assessment and classification of disease activity within 12 months (e.g., DAS28, SDAI, CDAI, RADAI, RAPID) 3. RA: Functional Assessment (PQRS #178) Percentage of patients aged 18 years and older with a diagnosis of RA for whom a functional status assessment was performed at least once within 12 months (e.g., HAQ, MDHAQ, HAQ-2, ACR’s Classification of Functional Status in RA)

Measurement and Reporting Each quarter, report on past 12 months Ideally, begin with baseline for 12 months ending 6/30/2014 Reporting for collaborative: 9/30/213, 12/31/2014, 3/31/2015, 6/30/2015, 9/30/2015 Detailed specs to be developed Age limit? Timing of Dx? Specific instruments for disease activity and functional status? Report via Excel templates, emailed to AMGA Progress reports will be circulated quarterly

Participants’ Responsibilities 1. Create an implementation team Must include an employed or clinically integrated affiliated rheumatologist Integrated EHR for all physicians and sites participating within your project Patient participation as a team member is strongly encouraged Up to 4 team members are encouraged to attend in-person meetings Develop process for measurement and reporting, and submit progress reports and measurement data on time. Participate in face-to-face meetings and all bi-monthly webinars.

Participants’ Responsibilities 4. Share tools, insights, and protocols related to the project. 5. Engage in open dialogue and discussion. 6. Submit the final data report to AMGF by October 2015 7. Participate in Best Practices Case Studies Agree to allow AMGF and AbbVie to publish information such as interventions, findings, and other supporting materials related to the project AMGF to provide medical writer assistance

AMGF’s Responsibilities Provide timely updates to participants. Be available for questions and/or concerns related to the project. Provide PowerPoint templates to participants to use for progress reports. Excel templates for data reporting, along with measure specifications. Serve as resource and manage the collaborative. Maintain the collaborative website. BE THERE FOR YOU!

Kick-Off Meeting Purpose & Objective Opportunity to network with peers in the collaborative & participate in shared-learning through group work Highlights each groups current work Provides educational content and sets the schedule of work Presentations AMGF will provided a presentation template for participants to use: 15 minutes maximum to present, including question and answer period Will present in small groups   Please email your 15 minute presentation to Danielle Casanova at dcasanova@amga.org no later than August 8th.

Kick-Off Meeting Reimbursement If selected to receive a grant, travel and hotel expenses are expected to be covered by the educational grant you receive. If not selected for a grant, AMGF will reimburse coach airfare and ground transportation up to $800, and two night’s hotel for two people. We ask that you cover expenses for additional team members.
