Onboarding Reference Guide Agency will Onboard first. Principal of the Agency must then go through onboarding as an Agent The Agency will then invite others in the downline to onboard Before beginning this process make sure you have the following available. This information may be required to upload during the Onboarding process. For New AGENCIES: - a copy of your Errors and Omissions Insurance Policy (E&O). - A PDF version of your W9 (can create on the spot in system if necessary) - A PDF version of a cancelled check or other bank documentation for the bank account that will be used in the Electronic Funds Transfer (credit union etc) For Recertification of AGENCIES: a copy of your Errors and Omissions Insurance Policy (E&O). Envision will send out the Invitations for existing Agencies and Principals to recertify RECERTIFYING AGENTS: - a copy of your Errors and Omissions Insurance Policy (E&O). - have any compliance certifications available (AHIP, FWA, Gorman training). - If you’re onboarding under your own agency you will need the Agencies tax ID number and NPN on hand. - If you’re commissionable and would like your commissions assigned to another agent under your upline you will need that agent’s NPN.

Agent Onboarding AGENCIES will determine which Agents will be commissionable or non-commissionable with the invitation they will send The appropriate Tabs will populate in the workflow
1 - Invitation Email The process begins with an organization sending you an email inviting you to onboard. Click the Login link when ready to begin Remember your DOMAIN will ALWAYS be ENRX
2 – Login The User ID in your invitation email is your Login Name. You will be asked to change your password on your first login. The value for the Domain field will prepopulate when you open the login page through the link in the invitation email, but you will have to enter it manually if you reach this page through any other link. DOMAIN is ENRX Enter your credentials and press Submit.
3 - Reset Password Create a new password to continue. Between 8-20 characters must contain both letters and numbers
4 – Open the Application Once you log on, you will see the application link Click the blue link under the “Application” column to open your onboarding application.
5 – Complete PDB Report Request Form If you are onboarding as an agent enter your SSN, otherwise enter your agency’s TIN, then select the next Tab button, or Enter button to advance. Your NPN number and NPN Lookup Results will automatically be retrieved. Authorize the NIPR request for your PDB report. Then click the SUBMIT button
6 - General Tab You’ll now be taken to the main body of your onboarding application. Many fields on your application will already be completed with the information returned from the PDB report. PLEASE VERIFY this information is still correct. You can change any of it only on this page. If you see an asterisk * by any fields, that means that information is required Your progress will be saved on tab change so you can complete the application over multiple sessions, and you can click the “Save” button at the bottom of the page to save a partially complete tab. You will begin on the General tab. Update and add information as needed, then select the next TAB, in succession. IF you omitted any required information on any of the tabs, you will see “incomplete” in RED will remain on that tab. Please go back and complete.
6 - General Tab continued There are multiple tabs on this page, most of them containing fields that must be completed, reviewed and confirmed. Start on the General Tab
7 - Licenses Tab Check the licenses of the states you are licensed in, that you plan to do business with Envision in. At least one license must be selected. Click the next tab, the Appointments tab
8 - Appointments Tab If you have any existing appointments with Envision in the states of your selected licenses, they will be listed here. This Tab does not require any input. It simply displays information For training purposes All tabs are showing here. In reality, only appropriate tabs will show for how you were selected to be On-boarded (such as commissionable or non-commissionable). Only the tabs each category will need, will be present For training purposes only, If you are onboarding as an agent, click the Background Questionnaire tab and go to next step 9. Otherwise, click the Insurance tab and go to step 11. *Remember only the appropriate tabs will be present for how you are to complete the workflow
9 - Background Questionnaire Tab Answer all background questions, providing explanations for each “Yes” answer in the box that will appear.
10 – Background Agreement Tab There may be several different type of agreements, depending on onboarding assignment. Click each agreement button to open them. Sign each agreement using your full name as given on the General tab and click the Sign button. If not exact as prior used on General tab, the name will be populated below the line for you to see how to sign. Click the SIGN Button after typing in your signature Next select the Insurance tab.
11 – Insurance Tab Provide all required E&O insurance information and upload a copy of your certification.
12. Banking Information First you have to select WHO is the PAYEE? Yourself, your Business, or You are assigning it to another person in your Hierarchy Fields related to your choice of Payee will appear. Complete these fields and upload a voided check or banking document Yourself as payee example

13 – W9 tab If you have a completed W9 in PDF format, upload it using the Upload button If you do not have a W9 completed, click the Open a W9 Document button to open an interactive W9 PDF. You do not have to print and sign it for it to be used
14 – Certifications Tab Select the radio button corresponding to the certifications you have; this will cause the relevant fields to display. Upload your certifications, including a completion date for each one.
14 – Certifications Tab continued If you have your compliance certificates through Gorman, separate FWA and AHIP documents need to be uploaded. Include the completion date.
15 - Agreements Tab Click on the Agreement buttons to open them. Sign each agreement using your full name as given on the General tab. Click the sign button when finished. If you sign your name differently, a direction will populate under the space letting you know how you signed your name. Name must be exactly the same. In the document after signing, select the SIGN button
16 - Education Tab Complete the Litmos training module(s) provided and take a short test. Must pass the test within 3 attempts with a 85% score If not able to pass in 3 attempts, must wait 6 months to try again.
17 – Submit Tab Once all required Onboarding information is entered, click the Submit Button Submit and Wait for Envision Approval, Background Check and Appointment confirmation. Once approved and appointed, the Agent receives an email notification that they are onboarded and given a login link to the ICM system where they can go to see their writing code.
18 – Success Page You will be able to review the information you have submitted. No information can be changed at this stage. Once Agent is onboarded, they will be able to edit their personal information
19 - Email Notifications You will receive emails notifying you if your application is approved, rejected, or requires more Information. After onboarded, you will receive email notification with your login to the ICM system to see your new writing code and access to the new Broker Portal. Broker Portal is where you will be able to follow your enrollments and see your commission statements if applicable.
