Performance Management Quick Reference How to conduct annual evaluations in the system: Open Performance Management website: Lifenet, Departments, Human Resources, Leadership Resource Center, Performance Management, Performance Management System or go to: https://performancemanager4.successfactors.com/login?bplte_login=1&company=StThomas& Login Username = User Email Address (CASE SENSITIVE) Password = last 4 digits of SSN Click on “Performance” Choose the form you would like to work on Click on the name of the associate (under Form Title) that you would like to review You will be directed to the review document of that associate. Give each objective a rating by clicking the rating drop down box and choosing Exceeds, Achieves, Partially Meets or Does Not Meet. It is suggested that you write comments in the comment section if you give a rating of Exceeds, Partially Meets or Does Not Meet. You can see the key actions for each Behavioral Objective by clinking on “Link” next to the definition. (This function will not work if you are not on the STHS network) Once you have rated everything click on “Save and Close” Open the form back up by clicking on the name of the associate (under Form Title). The form will have updated with an overall rating. When you are completely finished with the form click on “Send to Next Step” You will then be routed to another page that tells you “You’re about to submit this form for completion.” Click “Send to Next Step” When you have completed all associate reviews your Performance” page will be empty. All completed forms will now be in your Completed folder. Documents that must be submitted to the Ministry Service Center (MSC) on each associate: Signature page for Annual Performance Reviews Signed Standards of Conduct form Population Served form – Clinical Only Competency form – Clinical Only Please submit these forms to the Ministry Service Center Using the Document Imaging Cover Steet Updated 06.14.2012
