How the New Telephone & Appointment System Works! The system has to be changed to: to give easier access to your doctor; to minimise the number of wasted appointments and to ensure appropriate booking of face to face appointments. Call 01296 425775 – then choose the appropriate option from the following three: Option 1 – This puts you through to the Clinical Access Centre who will book you in for a call back from the doctor. Option 2 – For life style advice. You will be put through to Live Well Stay Well who can offer and organise support for you locally. Option 3 – To contact the surgery for: Pre-bookable telephone calls or appointments with your doctor Relaying a message to the doctor (not requiring call back) Booking nurse & midwife appointments To re-arrange post natal, child immunisation, long-term condition appointments (i.e. diabetes) & any appointments booked in for contraception (i.e. coil/implant fitting). Travel immunisations Referrals Medical reports Home visits Prescription queries Test results

Option 1 If you press 1 you will be directed to the clinical access centre who are able to request the doctor to call you back. For continuity of care it is always better to speak to your usual doctor when you can, especially for non acute matters. Below is a table to show you when your doctor is usually in, however, this may be subject to change on occasion.

Option 2 If you press option 2 you will be directed to Live Well Stay Well who can offer and organise support for stopping smoking, losing weight, being more active, healthy diets or even long term condition management locally. The initial phone call involves an assessment. Option 3 Finally, if you would like to speak to your surgery reception team about prescription queries, test results (after 2pm), nurse appointments, interpreters or any other matters, please press option 3.

Please note: Pre-bookable telephone appointments can be booked in advance by patients (subject to availability) Prescriptions can only be taken over the telephone for housebound patients (very frail or physically unable to leave the house) If you book an appointment on the app or website it will be for a telephone appointment Hospital transport will need to be arranged yourself – the telephone number to arrange this is 0300 100 0015. Please have your NHS number to hand.
