Community Living Program Assess/Identify outcome – change both how the goal is reached (self-directed) and funded (hourly to daily). Provide supports when needed/requested and minimize/eliminate “just in case staffing scenarios”. Support the entire person, not just a few needs. The cost to support 98 people who moved from 24hr staffed group homes The cost to support 98 people who moved from 24hr staffed group homes. -Annual savings of $3.8million as compared to previous service arrangement Group of individuals offered the choice of Community Supportive Living have diverse needs for support, ranging from relatively little to extensive and complex. This mix was important in order to learn what was possible in terms of cost and in learning if there are any limitations on who might be offered this option in the future. Daily rates range from $3/day to over $200/day as demonstrated in chart.

Community Supportive Living Cost Effectiveness The chart shows 98 members currently living in Supportive Living cost $61.54/day. The same individuals living in a residential setting of 1-8 beds with sleep staff would cost on average $77.03/day. CCCW would have spent 25.2% more ($554,077 annually) on the same individuals. The same individuals living in a residential setting 1-8 bed with awake staff would cost on average $107.03/day. CCCW would have spent 73.9% more (1.6 million annually) on the same individuals.

The results of new tools Innovative Services, Inc. was founded in 2004. In 2014 ISI became Wisconsin’s largest residential provider. Finding ways to overcome challenges of low population density, limited cell and internet coverage and mother nature in ongoing expansion in Northern Wisconsin Northwest WI Development began in June 2015. As of May 1st, 2016 ISI SUPPORTS 120 PEOPLE in community living programs in Northern WI-natural supports encompass 30% of regular support

Cost-Effective Ability Beyond Clapboard Program: For every $1 spent in year 1 of using technology they saved $3.99. For every $1 spent after year 1 they are saving $11.23. UCP of NYC: Prior to using SimplyHome technology, staff at one location were needed 24/7. Today, staff is needed 21 hours per week, saving them at least $3,000 per month. Wildwood Programs has seen $39,000 savings per person. Annually, that is $500,000 savings for 16 individuals.
