Community Living Program Assess/Identify outcome – change both how the goal is reached (self-directed) and funded (hourly to daily). Provide supports when needed/requested and minimize/eliminate “just in case staffing scenarios”. Support the entire person, not just a few needs. Wrap around services Each person shares goal to live a self-directed life as a full citizen of their community. Intense supports when needed
Living a Real Life Supports available 24hrs a day Teach how to self direct and access community resources vs. relying on paid supports Flexible schedules Most supports provided based on life routines not based on alerts or notifications Changing the role of a caregiver to a facilitator
Think Differently – Critical Elements of CSL Program Daily Rates-service model is based on a Group Home Service approach NOT SHC Communication of choice, appropriate response and care plan change The role of Natural Supports vs Paid Supports Partnerships with local landlords Comparative Rate analysis to Comparative Service
Assessment Determine gifts and strengths of individual Tool to capture outcomes of person Tool to determine cost to support person and the identified outcomes in days not hours How to use Natural Supports in the care plan Measurement of outcomes vs tasks Take a recovery model approach to service not a medical model Standard supports are to maintain current home/apt; build/create natural relationships as part of the services offered; use natural supports to provide outcomes; reduce rates as people develop skills and have strong natural relationships; do not measure service or supports in hours and do not limit what supports are provided
The cost to support 98 people who moved from 24hr staffed group homes The cost to support 98 people who moved from 24hr staffed group homes. -Annual savings of $3.8million as compared to previous service arrangement Group of individuals offered the choice of Community Supportive Living have diverse needs for support, ranging from relatively little to extensive and complex. This mix was important in order to learn what was possible in terms of cost and in learning if there are any limitations on who might be offered this option in the future. Daily rates range from $3/day to over $200/day as demonstrated in chart.
Community Supportive Living Cost Effectiveness The chart shows 98 members currently living in Supportive Living cost $61.54/day. The same individuals living in a residential setting of 1-8 beds with sleep staff would cost on average $77.03/day. CCCW would have spent 25.2% more ($554,077 annually) on the same individuals. The same individuals living in a residential setting 1-8 bed with awake staff would cost on average $107.03/day. CCCW would have spent 73.9% more (1.6 million annually) on the same individuals.
