Community Health Center of Lubbock The Outreach Department at the Community Health Center of Lubbock oversees many outreach and educational programs for our patients and the community. We have Diabetes Self Management Classes that incorporate the DEEP ( Diabetes Empowerment Education Program) which are overseen by a DEEP Master Trainer. Along with our DSM Classes, we provide a Nutrition Class in which we use carb friendly meals to demonstrate healthy cooking ideas and recipes. We measure health outcomes of our participants and celebrate each of their successes.

Prenatal Classes At CHCL we provide Prenatal Classes for new and expecting mothers and their support system. Classes are held in a 6 week series. We use the March of Dimes Curriculum and teach on topics such as prenatal care & healthy eating, stress during pregnancy & things to avoid, shaken baby syndrome, etc. The Prenatal Case Manager is a Community Health Worker who is trained to use the curriculum as well as connect the participants to the community resources they may need.

Step up Program for Children CHCL developed the step up program to help children reach a healthy Body Mass Index (BMI) for their age. A healthier BMI reduces children’s risk for obesity-related illnesses. America is faced with the growing epidemic of childhood obesity. Studies have shown that an overweight child is more likely to develop into an overweight adult. The Step up Program for Children focuses on encouraging healthy eating and exercise. The program is for children between the ages of 2 and 12 who have BMI’s that have increased at least two points over the last year and with a BMI above the 85th percentile for their age. The Community Health Worker works with the nutritionist to educate children on healthy food options ( ex., fruit smoothies) and the importance of physical activity such as building obstacle courses or using the WII to promote movement.  

Exercise Program At our Main Clinic, we have 2 exercise rooms for which adult patients, children and community members can exercise at no cost. CHW’s along with myself lead the classes. We offer Tai Chi under the supervision of Tai Chi Master Trainer Richard Leirer daily. Walking Away the Pounds and Zumba Classes are offered on Monday, Wednesday’s and Friday’s. Many of our participants are referred to the classes by CHCL providers who have identified them as being at risk for, or having a chronic disease. Our participants enjoy the group activities and have begun to walk up to 5 miles 3x weekly. We are seeing a decrease in AIC’s, Blood Pressure, Cholesterol and weight as well as other health meaures.

Northern Texas Community Health Worker Resource Coalition Mission: Promoting a unified alliance among Community Health Workers and Community Health Worker Instructors to share and gain knowledge through Information Exchange, Peer-Support, Professional Development and Continuing Education in the North Texas Region.

Coalition Members The coalition consists of 250-300 members from Regions 1, 2 & 3. We hold our coalition meetings quarterly and give each training program a opportunity to give updates and share community resources. We have CHW’s from various backgrounds who are Patient Navigators, Case Managers, Care Coordinators, ACA Certified Application Counselors, Patient Advocates and work for entities such as Federally Qualified Health Centers, Catholic Charities, WIC, 211, Local Hospitals, and Managed Care Organizations.

Trainings/ CHW CEUs The Northern Texas Community Health Worker Program has provided trainings from various partners / CHW Training Programs such as: DSHS ( THSteps, Teens, Sex & Law), West Texas AHEC (Behavior Change, Effective Leadership, Coalition Building, etc), TMF/ Fundamentals of Chronic Disease, DEEP Curriculum (Diabetes Empowerment Education Program, Road to Health. We were featured in the National Diabetes Education Program Newsletter as well as being recognized on the Senate Floor of the State Capital.

Events/ Volunteering The Coalition has volunteered for events such as: Back to School (over 1000 free backpacks, school supplies w/ free immunizations were provided to the community), Diabetes Self Management Classes / Nutrition Classes (DEEP Certified CHW’s assist), South Plains Homeless Consortium (CHW’s are members), Women’s Health Month (CHW’s participate in Go Red for Women), Cinco de Mayo (CHW’s volunteer to provide resource information), Men’s Health Month (CHW’s participate in a event where we provide free health screenings and prostate exams for men), Juneteenth (CHW’s provide community resources), 4th of July (CHW’s participate in the parade), KEEP Lubbock Healthy Event ( CHW’s participated in a event with the National Kidney Foundation to provide Free Kidney Testing to the community and resources). These are some of the many events we participate in.
