Medication Therapy Management (MTM) Overview for Pharmacy Technicians William Havins, CPhT

MTM Services MTM is a patient-centric and comprehensive approach employed by pharmacists (or other qualified health professionals) to improve medication use, reduce the risk of adverse events, improve medication adherence, improve patient outcomes, and reduce overall health care costs. The focus of MTM is on the patient’s disease state and complete healthcare.

MTM services history Medicare Prescription Drug, Improvement, and Modernization Act of 2003 (MMA) MTM included in Medicare Part D benefit Defined MTM as a drug therapy management program That may be furnished by a pharmacist Is designed to ensure that covered Part D drugs under prescription drug plan are appropriately used Is designed to optimize therapeutic outcomes through improved medication use & to reduce the risk of adverse events1

MTM services History, cont. Affordable Care Act (Section 10328), effective 1/1/13 Required targeted beneficiaries be offed a comprehensive medication review (CMR) The Part D quality improvement requirement of 42 CFR 423.153(d) states a Part D sponsor must establish a MTM program that: Ensures covered Part D drugs are used to optimize therapeutic outcomes through improved medication use, Reduces the risk of adverse events, Is developed in cooperation with licensed and practicing pharmacists and physicians, May be furnished by pharmacists or other qualified providers.

Medicare Part d enrollee eligibility Multiple Chronic Disease At least 2 3 is the maximum minimum requirement Part D sponsor may require for targeted enrollment Taking multiple Part D drugs 2 is the minimum number of drugs Sponsor cannot require a minimum above 8 covered drugs Likely to incur annual costs for covered drugs exceeding a predetermined level The 2017 MTM program annual cost threshold is $3,9192 • Alzheimer’s Disease; • Chronic Heart Failure (CHF); • Diabetes; • Dyslipidemia; • End-Stage Renal Disease (ESRD); • Hypertension; • Respiratory Disease (such as Asthma, Chronic Obstructive Pulmonary Disease (COPD), or Chronic Lung Disorders); • Bone Disease-Arthritis (such as Osteoporosis, Osteoarthritis, or Rheumatoid Arthritis); • Mental Health (such as Depression, Schizophrenia, Bipolar Disorder, or Chronic/Disabling Mental Health Conditions). Sponsors may also offer MTM services to expanded populations of beneficiaries who do not meet requirements – not part of Part D coverage.
