NNPD Metabolically-Challenged Cohort Study Ethics proposal submission: 19th May 2008 Estimated start date: September 2008 Estimated duration of study: 36 months

Study Objective To create a component of the national nutritional phenotype database that represents the metabolic stress from varying body-weight overlaid by the additional metabolic stress of a test meal

Glucose Fat Δ 1 4

Develop central hypotheses (examples) - Effects of a metabolic challenge on the metabolomic profile. - How do metabolomics profiles change with time in the post-prandial state? - Is clustering of baseline metabolomic data still evident in the post- prandial data? - Can we identify a metabolic phenotype with respect to post-prandial response using baseline metabolomics?

#1. Options for Study Design 600 x 1 as in the grant or 200 X 2 + 200 x 1 or 100 x 3 + 100 x 2 + 100 x1

#1. Options for Study Design 200 subjects visited once + 200 subjects visited twice = 600 EVENTS 0 1 2 3 4 RECRUITMENT CONTROL DIET CLINICAL INVESTIGATION MORNING (CIM) 200 Norm (Measurements + Test Drink) 200 Overweight 200 Obese Phase 1 Phase 2 0 1 2 3 4 5 RECRUITMENT CONTROL DIET CIM CONTROL DIET CIM 100 Norm 100 Overweight 100 Obese

#2. Inclusion & exclusion Wide inclusion, narrow exclusion preferred

#3 ~ Recruitment Wide: Email/ Companies/ Hospital/ GP practices… - Males/females 18-60 yrs (focus: 25-45yr) - Wide BMI range 18.5 – 45kg.m2 200 Norm 200 Overweight 200 Obese - “Healthy” (NO medication, except for contraceptive pill) …

#4 ~ Test meal Test meal Test Meal 1 only or two Liquid (preferred) Solid Mixed

#5 ~ Measurements: Clinical Investigation Morning Transport Volume Analytes Frequency Storage Collection Processing

#6 Control Diet Objective: To minimise variability between subjects prior to starting. Weight should remain stable during this period Options: Total 4 week run in control Provision of a list of “authorised”/ “non-authorised” Foods and Drinks (Preferred) No control
