Frailty Fulcrum Jane to lead To research, design and deliver a multi-agency, multi-professional toolkit to support and enable health and care professionals to take a consistent and evidence based approach to supporting self-care for older people living with frailty. To develop an innovative education and training programme to launch the toolkit to participants across Nottingham City and Nottinghamshire. 8 Whole-day Education & Training events for cohorts of 36 – 50 across Nottinghamshire Offered to all health care provider organisations involved in health care and support services for older people with frailty The target audience health and care professionals working in Agenda for Change Bands 2 to 7 (or equivalent roles for non- NHS organisations)
