Our vision To bring discovery and innovation into the Wessex health system so that the population has better health and benefits from a thriving health innovation sector   We aim to: Ensure that people across our region can expect the same, high quality of care regardless of location or provider through collaborative service improvement and innovation programmes. Rapidly translate research into practice, promote adoption and spread of innovation – including timely implementation of NICE technology appraisals – through efficient knowledge exchange.

Making Primary Care Surgeries Dementia Friendly Identify a Dementia Champion In Tower house it our HCA who is wearing pink, has inspired senior nurses to attend DA training, and is piloting a memory app Staff who are skilled and have time to care Regular all Island training for GPs and nurses Staff training down at Tower house Partnership working with carers, family and friends Identify and phone carers (don’t ask them to attend surgery) they cant leave their loved ones . Assessment and early identification of Dementia Offer people who are concerned a formal assessment, take their concerns seriously Care plans which are person centered This is me from Alzheimer's society ,Advanced care planning Island wide Environment s that are dementia friendly Good lightening bright colours, toilet doors painted a different colour. clear signage

How did Tower House Surgery become Dementia Friendly ? Semi rural deprived area 10,500 patients 25% Island population over the age of 65 Higher percentage with dementia Started with myself being CCG clinical lead for dementia Offered opportunities to provide support for various projects My HCA became an informal dementia champion All staff under went dementia awareness training Nursing staff attended Dr Gemma Jones dementia training

Case study sites – Oakley/ Overton surgery and Ryde surgery Methods of Evaluation Case study sites – Oakley/ Overton surgery and Ryde surgery Interviews with GPs and staff – Normalisation Theory tool Patient and carer experience collected Routine data collected from surgeries, NHS primary care web tool and small survey data

Oakley/Overton Feedback from patients and carers Staff have good understanding of dementia We feel listened to Carers feel included and valued Patient – “feels more confident now and that has helped improve things” Signage Receptionist forgetting to tell carers about appointments Anticipatory care Long waiting times in reception is difficult for carer when accompanied by a person with dementia

Ryde Feedback from patients and carers All staff have good understanding of dementia issues Big improvements from 5 years ago when there were no dementia services after diagnosis Excellent care from GP for both patients and carer More leaflets in waiting room Regular GP initiated check –ups More availability with 1 named GP More social support needed

When doing a refurbishment think “dementia” Next steps This initiative has already been spread to USA, NZ, Norfolk, Nottingham and is starting across Wessex – 10 surgeries in train A bundle of actions is recommended, e.g. Identify a dementia champions, involve patients and carers and tailored training depending on role When doing a refurbishment think “dementia”
