1. All about me 2. Family and Friends e.g. photo of child Name a nickname if appropriate Date of birth Where I live (no address, general area e.g. town.) Some parents may wish information on child’s diagnosis. It may be nice to include photos on this page. Remember to include pets if appropriate.
3. Things I like 4. Things I like to do It might be good to include photos of TV characters, videos or music your child enjoys. This may include places your child likes to visit or activities they like to do.
6. Things I don’t like 5. Important things you need to know about me What would you like new people to know about your child to ensure a successful interaction. e.g. If I am really enjoying an activity I find it hard when you tell me we have to finish. I will need some warning that one thing is about to stop and another to start. Photos and symbols really help me to cope with this. I use a communication aid called a Tobii. I might need some extra time to tell you things but don’t worry it’s worth the wait. You might want to put in things your child doesn’t like to hear as well as activities she/he doesn’t like to do.
8. Things which might upset me 7. How I work best 8. Things which might upset me You might want to put how to comfort me when I get upset. Include any routines I have, particularly if I have a special way to do them. School and nursery may also have ideas about this.
10. Things I like to talk about and things you might want to talk to me about 9. Eating and Drinking Contact SLT if appropriate, otherwise this may include food and drink preferences

Things I am learning about These would be general things and not specific activities. e.g. I am learning to use my communication aid I am trying to remember to watch my distance when I meet people – sometimes I forget and get too close I am learning to ask for help when I get stuck It is probably useful to contact your child’s OT/ Physio/ Sensory Support Service about this if appropriate.
helping me to communicate 13. How I communicate 14. Do’s and don’ts when helping me to communicate It is probably also useful to contact your child’s SLT and school/nursery about this. Contact SLT for advice in completion of this page.
