CASE OF Dyspepsia & Amoebiasis By Dr. Jawahar Shah M.D.(Hom)

A 43 year General manager of a big company with severe pain in the abdomen

The complaint started after taking Erythromycin tablets for a throat infection about 10 months ago

Severe & constant pain in the abdomen & epigastric region Mucous in the stool. ++ Had taken treatment for amoebiasis without relief

Constantly feeling hot in the abdomen Gets up in the middle of night Keeps tossing in bed due to the sensation Has to rush to toilet immediately on eating

Anxiety about illness ++ The anxiety about the pain was so well marked that even the thought of pain would produce jitters Due to this tremendous anxiety, he would develop anorexia & lose weight

Pain had no specificity No diagnostic relevance All investigations were negative Fears that he would die because of sickness

Other Complaints Backache worse by Sleep during First motion Better by Daytime Past History Typhoid Rheumatic fever

Family History: Father: IHD; Acidity Mother: Facial paralysis Sister: Valve replacement. Expired.

Personal history Appetite: Anorexia since five days Does not feel like eating due to pain Thirst : +++. Frequent. Glassful at a time

Perspiration: Scanty. He hardly perspires Thermal: Prefers cold weather, but cannot tolerate cold. Must cover himself, even in summer

Dreams Of office matters Business about Buying a Flat Sleep: Disturbed only because of pain

Born in a village, in a middle class family. As the youngest in the family he was pampered by his parents Came to Bombay after his B.E. started working as a manager in a reputed company

Anxious about his work. He is anxious about his health + + Very anxious about illness. Restless during pain Cannot feel comfortable in any position when in pain

Basically irritable by nature Gets disturbed very easily, even if the telephone rings often He prefers to be in company and always likes to meet people

On Examination: Wt. : 60 Kg Black circle around eyes Tongue Coated Teeth Caries. Hair loss ++ Graying of Hair ++ Hyperextensible Fingers BP - 148/100 Upper part of body emaciated

10th February SILECEA 30 5 PN SL 1-0-1 x 7 days SL 2 pills every 4 hrs

17th February Sleep better + Burning decreases + Constant pain in abdomen Change in colour of stool from black to yellow Continue all x 15 days

25th February After 15 days Pain SQ Continue all x 14 days

14th March Pain SQ No major improvement Burning in Abdomen Worse by night & lying down Gets up in the middle of sleep with pain Restlessness with pain Not comfortable in any position Weight loss Ars. Alb.200 1P now. SL 2 pills every 4 hrs. SL 1-0-1 x 14 days

26th May Much better Mild pain sometimes Continue all x 1 month

After 3 Months Changed his eating habits Started taking spicy food, but since 2-3 days very mild burning pain Ars alb 200 1P stat SL 1-0-1 x 2 months SL 2 Pills every 4 hrs

20th August Nearly after 1 1/2 year interval, patient developed the symptoms again, but not with the same intensity or anxiety

Learning Physician must be open to the symptomatic evidence expressed by the patient. If your remedy is right it has to produce relief, you should not have any hesitation in changing it.

If sensitivity of patient is right he will give response immediately at least one of the levels gets relieved Physically Emotionally Intellectually Sub consciously

If patient is suspicious or anxious he may not take your prescribed medicine as advised. In these circumstances give 1 dose of indicated medicine right in front of you on the patient’s tongue.

This will allow you to be sure that the patient has at least received the right constitutional medicine to start with.
