Sustainability and Transformation Plan Herefordshire & Worcestershire

Sustainability and Transformation Plans: purpose and scope To work together across a larger footprint to make joint plans for local health and care services – a ‘sustainability and transformation plan’. The idea is that some of the challenges being faced could be better and more effectively solved in partnership with neighbouring economies on a larger scale, with the support of local communities. The footprint we are working on is Herefordshire and Worcestershire. We are developing a plan to bridge the gaps we know exist in relation to Health and Wellbeing, Care and Quality and Funding and Efficiency, known as the triple aims Plan will be submitted at the end of June, although the final submission is likely to extended to allow further work to be developed.

As part of the Herefordshire and Worcestershire plan we will need to consider: How we can help to reduce future demand for services by helping people to live more healthily and independently What services make the biggest difference and which we need to invest in. What we can do differently What we need to stop doing altogether because it is not a priority

Clinically safe services I would be prepared to travel further than I do now to access healthcare more quickly. I would be prepared to travel further than I do now if the healthcare provided further away was safer or provided the best clinical outcomes. Home first I would prefer to avoid having to be admitted to hospital for care if I could be safely treated at home I would prefer to pass away in my own home if at all possible rather than in a hospital Resilient communities I believe that I have a responsibility to care for my own family members as they get older / my family have a responsibility to care for me as I get older. I accept that in future I may need to help provide some care to friends and neighbours to help them remain independent and avoid the need for hospital stays. I need to take responsibility for my own health and well-being and I should do all I can to avoid the need to be treated for conditions that could be avoided if I was more responsible. I would be happy to be taught to self-care for my aspects of my condition where it is safe to do so.

The right professional I would be happy to be seen by the most appropriate professional, even if that wasn’t a GP. I would not complain if I was denied access to a health service if somebody explained why I should use a different service Better use of technology I would be happy to access some healthcare services by alternative means such as telephone, email, Skype/FaceTime etc. rather than seeing somebody face to face Enabling change The local health and care system needs to be better organised to enable real and sustainable change to happen We need to reduce duplication, make it easier to access the right service, first time, and we need to overcome organisational divides to ensure collectively we are always doing what’s right for the patient
