So what’s with the new name? Grant funders saw us as old fashioned and just providing infrastructure so didn’t want to invest Went through a review of why we existed and what difference we wanted to make to the local community across Glossopdale Went through an internal restructure and stopped doing some roles and looked at how we could reposition and reinvent ourselves Used our reserves to help transform Hard year maintaining Companions whilst and setting up new services

Integrated Self Care in Glossop Investment brought together from Tameside and Glossop ICFT, Derbyshire County Council, DCLG Communities Fund, Big Lottery and Bureau reserves Four interdependent strands: Social Prescribing and Local Area Coordination Creating a new volunteering / community connection offer ABCD Community Development Grant funding and spot purchasing

Integrated Self Care Vision Support/investment for a Market place of self help groups and self care support services ‘Super’ Volunteers Community Navigation

Snap shot of referral demographic so far

Our target client base Conditions that put the most strain on our services We will also consider At least 1 long term condition Hypertension Diabetes mellitus Depression COPD Cancer Heart Failure CAD Asthma A debilitating injury An unpaid carer A disability Mental health issues Social isolation, lonely or struggling to cope Experiencing low self-esteem, loss of confidence, low-mood or bereavement

The ‘market place’

The Market Place Need to know what services and activities are available for people to access Where there are repeated gaps locally we will use mix of infrastructure support, peer support and grant funding to set up new services Where we are significantly increasing demand on existing groups and services we will use grant funding to help build capacity Where there are particular one off barriers to helping people move on we have the option to spot purchase one off services

Super Volunteers

Super Volunteers Co-produce the ‘Super Volunteer’ offer with existing volunteers and partner organisations Short term and ad hoc roles that connect people in the local community Resource that works across community and primary care Volunteer Brokerage offer now through The Bureau Website Proactive role setting up and supporting others with community activities that bring people together

How is it Helping Individuals?

Challenges for Glossop’s VCS Grant funding needs to address the following outcomes: Reduce repeat demand on GPs where the causes are due to social factors, rather than medical factors Reduce inappropriate referrals to ASC Speed up the rate of discharge from hospital Reduce unplanned hospital admissions Need a new language to describe what we do Need to understand our unit cost Need to track back to effective preventative interventions Need to promote what is already available in Glossop

DEVELOPING GROUPS High Peak CVS have a track record offering development support to voluntary groups Support is now also about helping groups to adapt into this new community approach What can groups offer to support self care?

Support to Change Groups offering help to new people What changes are needed? What can help groups be secure, effective and able to grow? How to fill gaps where no group exists? Showing results and impact

Questions to you What is the best support your group has had in the past 3 years? … who from? What type of support will your group need to be part of this new approach in the next 3 years? Answers on a post it!
