We have found that we need to focus on Call Management and Care Coordination as 1st value streams, as, together, they provide the infrastructure for effective and efficient patient care. All of our care of patients happens through EPIC, so we need to standardize documentation and communication practices in EPIC as a foundation before optimizing other aspects of patient care. © 2008 VIRGINIA GARCIA MEMORIAL HEALTH CENTER

Care Team Whiteboard: Purpose and Expected Outcomes The Whiteboard is a Visual Management Tool for organization and communication of the work of the Care Team. We anticipate implementation of the Care Team Whiteboard to achieve the following: Balance the workload across Care Team staff Ensure timely completion of care coordination work (e.g. In-Basket messages, forms, faxes, etc.), getting information to patients when they need it Facilitate communication between team members Improve staff satisfaction The Whiteboard also serves as a vital tool for guiding Clinical Team Leads through their daily standard work. Whiteboard and CTL is bringing a lot of this discussion to light. Previously, team members in their silos. Not much communication. Assumption that, when someone is out, there are other coverage models at the clinic level that are taking care of it. Team members not being engaged to help cover for their own team member.   Efrain says that whiteboard helps him stay focused on his work and what his priorities should be.
