Hope, Agency & Opportunity Thinking about your recovery & what matters to you

WORKING RELATIONSHIPS The Recovery Themes Hope is about seeing a future for yourself Believing that things will get better Having things that you want to do HOPE Agency is about feeling that you are able to take control of difficulties your life Having choice & information about your care Knowing how to keep yourself well AGENCY Opportunity is about developing and supporting your strengths Supporting the role that you already have e.g. family member, student, job role Having the chance to get involved in your local community OPPORTUNITY Working relationships is about having people that listen to you and that you can trust Having people around that believe in your recovery Working together to build a care plan that fits you WORKING RELATIONSHIPS

Starting Points “Recovery is about building a meaningful and satisfying life, whether or not there are recurring or ongoing symptoms or mental health problems”. If you are struggling to have a conversation about the Hope, Agency & Opportunity PROM, here are some ideas that might help you get started. They are all home grown ideas, things that have proved helpful to service users and staff here in Southern Health who are working towards recovery. There is a template you might like to try out, it builds a big picture of what each service user’s recovery might include. “The information you write will help us get to know you, and develop a care plan that is right for you.”

What has helped me in the past? Who would I like to be involved? What does hope mean to me? What do I enjoy?   HOPE What is important to me?   What has helped me in the past? What does a bright future look like for me? (Activities? People?)   What are my goals?   I believe the difficulties I face can be managed or overcome? Who would I like to be involved?

Do I know what the alternatives are for supporting my wellbeing? What does having control of my wellbeing look like and feel like to me?   AGENCY What can I do to keep myself well?   Who else can help me? What can staff do to help me have a sense of control   What information do I need to make choices about my care?   Do I know what the alternatives are for supporting my wellbeing?

What does opportunity mean to me?   OPPORTUNITY Can I get the support I need to help me in my role as….. a parent, partner, or other family member? a friend or companion? an employee/student? What am I good at that makes me feel better?   What does opportunity mean to me? Is there something for me in my local community?   What support do I need to access opportunities?   Who else can help ?

What does a good supportive relationship feel like?   How can we work together to support your care and recovery?   What is helpful/unhelpful?
