Registering for patient access First log onto : patient.emisaccesss.co.uk. Alternatively download the patient access app onto your tablet or smartphone. ** ** Please note an internet connection is required to use patient access.

Registering for patient access On the left hand side of the screen you will see the green “Register” Button. Click this button to continue.

Registering for patient access On the next screen you will be asked If you have received registration letter. This can come by post or by E-mail. If you have click “yes” if not click “no”. The letter /E-mail will contain details such as linkage key ODS Code and account ID.

Registering for patient access If you clicked “No” to having a registration document, you will be prompted to locate the surgery Via the post code. Type in Danebridge’s Post code (CW9 5HR), then click next.

Registering for patient access You will then need to click on the surgery name. This is shown on the left hand side. Then click next Don’t worry about clicking on the map as this is just for reference.

Registering for patient access Once the surgery has been selected you will be asked to key in your details or those of the person you are registering. We recommend your password contain at least one capital letter and one number. Be sure to read and tick the “I agree to the terms and conditions” box before clicking next.

Registering for patient access Once you have completed this you will be asked to confirm contact details and security questions. These will be used if you ever forget your password. Please ensure they are personal to you and easy for you to remember.

Registering for patient access If you clicked “yes” to having your registration document, you will be prompted to key in the details from your registration document. Each box matches an entry on the letter/ E-mail.

Registering for patient access The linkage , ODS and Account ID are all in your registration document. The linkage key goes in the first box, the ODS code in the second and the Account ID in the third. The registration document will not have your NHS number. If you need this please contact the surgery.

Registering for patient access Once you have completed the progress you will see the below message. Don’t worry about remembering your account ID as this is the same as in your registration document. Registering for patient access If you are unsure if you have received your registration document details please contact the surgery. Please be aware that a signed patient access consent form is required and this can take 4 working weeks to process. The E-mail will be titled and the letter will be addressed to the patient seeking to register. It will contain details such as linkage key and user ID

Registering for patient access If you need to sign a consent form please ensure you come into one of the surgeries and bring with you some valid photo ID. If you do not have any, please contact the surgery before coming in. If you believe you have already signed a consent form but still don’t have access to all the services please check with the surgery before coming in.
