“ I was struggling to keep on top of my diabetes. My Health Advocate nurse made it easy. ” Robert was recently diagnosed with type 2 diabetes and found it difficult to monitor his blood sugar, make and keep his doctor visits, refill his medication and get his recommended tests and screenings. He also struggled to eat right and stay active. When Robert called Health Advocate about locating a provider, his Personal Health Advocate recommended he speak to one of the nurse coaches about his diabetes. Through regular coaching sessions, Robert talked about his roadblocks and set his own health goals. As a result he developed a diet and exercise program that fit into his schedule. He made time to see his doctor, refill his medication and get his tests as needed. Now Robert feels more in control and knows that he can always call if he needs added help or support.

The Member Journey Robert was recently diagnosed with type-2 diabetes and was having difficulty staying on top of his medication schedule, checking his blood sugar, keeping appointments and sticking to his new healthy lifestyle. Through regular coaching sessions, Robert found it was easy to set daily reminders to test his blood sugar and take his medication. 4 Robert called Health Advocate about locating a provider to help with his diabetes. 1 His Coach also gave Robert healthy recipes and worked with him to develop an exercise program that worked with his schedule. 5 His Advocate recommended he speak to a nurse coach after seeing he was overdue for a recommended screening. 2 After a few weeks Robert felt that he had better control over his diabetes, was eating healthy and lost a few pounds all on his own. 6 His coach worked with him to identify the issues keeping Robert from being on top of his condition. 3 Robert called again after his doctor visit to tell his Coach his doctor was amazed with the results. 7
