Facts in a Flash Each year in middle school students complete 25 hours of student service learning which is a requirement for high school graduation. Students will fulfill their 25 hours of student service learning through their Outdoor Education experiences and through participation in lessons presented by North Bay at Bonnie Branch Middle School before and after the trip.
Required Paperwork All forms are due between September 11 – 15, 2017 Permission Slip (required) Payment Form (required) Cabin Wish List (required if attending) Medical Information Form (required) Parent/Guardian Consent and Release Form (required) Medication Form Only necessary if medications will be needed Prescription Over-the-counter Must be completed by the doctor All forms are due between September 11 – 15, 2017 Permission Slip/Payment Form/Cabin Wish list are located on the same sheet (front to back) The Cabin Wish List is on the back of the payment form, Please read everything carefully. If you turned any forms in already besides the chaperone sheet, teachers gave them back to students.
Trip Financing Total Cost of the Trip: $165.00 Fundraiser: Joe Corbi’s Packets distributed on August 31st Orders must be submitted between September 1 – 11. Each item sold will earn the student $5 toward defraying the cost of their trip Pick-up date: October 25 from 3-6 pm in Room 38 September 14: Balance sheets will be distributed based upon Joe Corbi sales. Balance payments must be submitted Sept. 11 - 15
Trip Financing There are options for payment of the $165.00. If you receive FARMS assistance, please contact Dr. Drew Cockley, BBMS Principal, ASASP!!! Paying online is suggested and preferred
Chaperones 15-20 chaperones are needed during the whole trip. If interested, complete the chaperone form and send it in to your Ms. Cavanaugh on or before by September 15. Chaperones will be notified and confirmed before September 27th. No charge for chaperones.
