FMP Contract FMP contract transitioned from Tiber Creek to Arrowpoint on March 10, 2016 Other applications on this contract include: SMMS, RSP, RMS, GIMS, etc.
Access Levels Access granted by NGB Access level controlled Admin/NGB level SFPD – State-wide view access and approval authority FACC – State-wide access and approval authority FAS – State-wide access with no approval authority User must have a CAC (with NGB sponsor) CAC must be authenticated through AKO
FMP Links Live Site Training Site https://smms.army.pentagon.mil/GFMS Users must attend all FMP training sessions prior to being granted access to the live site Training Site https://smms.army.pentagon.mil/GFMStraining The same security measures are set up on the live and training site. Therefore, users must have a CAC to access the training site

FAC Overview for Military OneSource Types of Records People/Organizations/Units Interaction Tracking Service Member Family Member Community Resource Unit Quick Trackers Mass Communication Outreach Contact Cases Events The left hand column shows the individual or organizational records in the system (i.e. SM or resources). The right hand column displays the record types where interaction/activity has taken place with an individual(s) or organization(s). For example: when a FAC staff member interacts/assists/networks with an individual/organization from the left column, they create a record of the interaction using the most appropriate record type from the right hand side column. Service Member (SM) Record Only Army National Guard Service Member data is updated daily from the G1 Data Warehouse G1 data goes back 10 years If a SM separated/retired from the Guard over 10 years ago, the system will no longer be updated with G1 information Other Branch of Service information is manually entered by FAC staff
Service Member Record Includes Personal Military Name Date of Birth Address, City, State Contact Info Family Members Unit Rank Deployment Status Specialty Listing (i.e. dual military) Additional Family Management Outreach Cases Contacts Remarks Documents
Family Member Record Family Member records and updates are created and updated by FAC staff Records are listed under a SM record Records include: Name / Date of Birth Address, City, State Contact information Relationship to SM Specialty listing (i.e. Primary Contact, Special Assistance, Survivor)
Community Resource Record A Community Resource record is created when a FAC works with or networks with an organization or individuals who is providing assistance to military members and their families. Name of Organization/Individual Contact information (phone, email, web-sites) Location Service Area Type
Unit Record ARNG Unit Records are generated by the G1 Data Warehouse. Non-ARNG Unit Records must be manually entered This record type has not yet been fully developed Unit Name UPC/UIC Address, City, State Unit Info Unit Manning Unit Events
Quick Tracker Record A Quick Tracker (QT) is a record created to track informal information requests which do not require follow-up, detailed research or assistance (i.e. quick responses to non-emergent needs) Categories Information/Materials Branch Deployment Status
Mass Communication Record A Mass Communication is a record created to track informal information that is provided in mass via newsletters, emails, auto phone messaging, texting, or social media and is related to the Six Essential Services Method Content Recipients Distribution Counts
Outreach Record An Outreach is a record created when well-being contact is made to a Family Member(s) who has a Service Member deployed or mobilized over 31 days. The record includes the range of dates where well- being contact is made on a monthly basis. Timeframe Method of contact Actions (i.e. successful two-way, attempted, no contact)
Contact Record A Contact is a record created when non-deployment related contact is made to Service Members, Family Members and Community Resources, with any frequency, for reasons not related to Outreach or Cases. Type (differ for SM/FM and Community Resource) Timeframes Methods of contact Actions (i.e. response pending/received, ongoing, complete)
Case Record A Case Record is created when a Service Member or a Family Member has an issue or situation and has asked for assistance. Record includes documentation of the issue – to include historical actions - taken to resolve or close the issue. A separate case is opened for each issue, under the name of the person who benefits from the services. Categories and Types Referral to Community Resource(s) Actions (i.e. follow-up, closed)
Event Management Record An Event Record is created when FAC staff host or attend an event. Events include, but are not limited to, meeting with commands, drill weekends, family days, job/employment fairs, community networking, etc. Name of Event Type Intended Audience Method Event Staff Attendance Event Details

FMP Main Screen – Command Center FAC Overview for Military OneSource FMP Main Screen – Command Center Ability to search the following: Service Members Units Family Members Events Community Resources The Command Center is located on the left hand side of the screen in the first section or “panel”. Example of a report you can pull in this area: SMs who live in a specific city or county. Number of youth ages 12 to 18 in your state. Resources who provide financial aid. Developed: March 2016

FAC Overview for Military OneSource FMP Work Buckets Displays a snapshot of Cases, Events, Outreach, and Contacts by access level. SFPD: entire state FACC: entire state FAS: their own work The work buckets give you a snap shot of what is happening – in real time – in your state. For example, you can see how many cases are currently being worked, how outreaches are progressing during the month and how many events are taking place. When you click on any of the line items, they will display in the search results (2nd panel). Developed: March 2016
