Mandatory Athletic Participation Forms Mandatory Athletic Participation Forms: Parents, for the 2016-17 school year, the required athletic participation paperwork for the UIL and PISD will be completed using electronic forms handled via our Rank One software. All incoming athletes in 7th-12th grades are required to have these forms filled out prior to practice and UIL competition. Please follow the link https://prosperisd.rankonesport.com/Main/Default2.aspx?Type=4 and complete the online forms as soon as possible. (Before Aug 22!) There are instructions on how to complete the forms on the Rank One. webpage the link takes you to. The physical and medical history forms will be the only paper forms to print either from the Rank One site. The physical form will still need to be submitted to the campus your child will be attending prior to any activity.  If you have any questions please email our Head Athletic Trainer at PHS, Michael Henry, at mdhenry@prosper-isd.net.

First mandatory grade eligibility date is September 30th. During the period of ineligibility Athlete will practice, but will not be able to participate or travel to games. Athletes may regain eligibility at the 3 week period if they are passing ALL classes.

PISD Athletic Trainers Coaches need to be notified if an athlete is injured during practice, game, or in other activities if it will interfere athletics. Michael Henry or Sarah Hartman, PISD trainers are available before and after school for consultation and treatment for injuries. The trainers are located at the indoor facility at Prosper High School. A doctor or trainer note will be needed if the injury prevails. Injured athletes are required to be at practice and dress out. Practices will not have to be made up if the athlete is paying attention and engaged in the practice plan. Prosper I.S.D. has a supplemental insurance policy that covers our athletes. ~Athletes/parents must file on their personal insurance before any claims will be paid by the supplemental policy provided by the district. ~THE SUPPLEMENTAL INSURANCE DOES NOT GUARANTEE THE ELIMINATION OF "OUT OF POCKET” EXPENSE. Parent Meetings All coaches are more than happy to meet with parents to discuss your child. Coaches will not discuss playing time or other players. Parents should encourage players to talk with coaches prior to parent meetings if there is a concern. Parent meetings will be scheduled. We will not meet with a parent after the completion of a game. Parents are welcome to call/email to set up a meeting. Athletes are encouraged to attend meetings with their parents.

Rogers MS Boys Athletics Consequences Late to Practice: Designated coach will coordinate extra conditioning that day. Extra conditioning will be gassers (or conditioning appropriate for sport). First time late = 2 gassers, Second time late = 3 gassers etc.. Process will start over on the day after the game that week. NOS = Not our Standard. Missing practice: (Unexcused) Athlete will sit out one quarter of the game that week. For 2 unexcused absences player will miss 2 quarters (one half). For 3 unexcused absences player will miss the entire game for that week. Missing practice: (Excused) Present Doctors note to Head Coach. Playing time in that week’s game will be affected due to limited amount of practice time. Tutoring in place of practice: Player must communicate tutoring in advance to coaches and player must have that teacher confirm their attendance. Try and schedule tutoring opposite/around practice time. Doctor’s appointment scheduled during practice: Player will give his Head Coach a 12 hour advanced notice for the practice they will miss. Excused absence may result in limited amount playing time for the game that week. Present doctors note to Head coach. Stealing/Theft: Follow policies and consequences set forth in 2016 – 2017 Eagle Athletic Handbook. Please read handbook thoroughly. ISS: Will result in NOS (Not Our Standard). Will result in extra conditioning and a loss of playing time in that week’s game. Extra conditioning will be gassers. First ISS = 2 gassers, Second ISS = 3 gassers, Third ISS = 4 gassers etc. Process begins fresh after game that week. ISS on game day results in loss of playing privileges.
