What is Connected Nottinghamshire? A Programme of work supporting Vanguard(s) and transformation (through Information Management and Technology enablement) across Health and Social Care in Nottinghamshire

What is Connected Nottinghamshire? Information Governance Records and Information Group well established County wide consent model for direct care agreed and in place Interim arrangements in place for Secondary uses of data Proof of concept complete for whole system reporting

Progress Access to information systems in interim phases now complete Medical Interoperability Gateway (MIG) implemented across all U&EC providers eHealthscope and GPRCC project in phase 3 Nottinghamshire wide “Health and Care Portal” implementation underway and live at Nottm University Hospitals and Sherwood Forest Hospitals Social Care information now exchanged daily and on demand

Moving forward Vanguard foundation & lessons supporting system wide delivery Local Digital Roadmap (LDR) is the new five year plan Workstream 1: Information sharing Workstream 2: Infrastructure Workstream 3: Citizen access to information and care records Workstream 4: Digital maturity (How good are our systems and how well are we using them) Workstream 5: Assistive technology Nottingham looking to move at significant pace to support the STP (Technology Enabled Care is a high impact area) LDR is an opportunity to finance some of the integration work (at least up front and initial running costs)
