Requirements for Participation Complete Life School Athletic Packet Online (you will receive information in the mail over the summer.) Completed & Cleared Physical Online Pay Athletic Fee--$75 per student ($25 for each additional child) per year (MUST BE PAID ONLINE. Information will be sent in the mail over the summer) All forms/fees must be completed by the first week of school Packets handed out Tuesday and Wednesday Physical must be within one calendar year CareNow Ten Clinic Fee can be paid with cash, check, money order or credit card Payment arrangements can be made with Coach Thrush Forms must in by the first week for ALL sports, Fall/Winter/Spring

Requirements for Participation (cont.) Athletic uniform (shirts/shorts) are available at our uniform vendors and online (Information will be updated on www.lifemustangsathletics.com and will also be sent home in the mail over the summer). Some sports may require additional equipment (i.e. kneepads for volleyball, cleats for football) Athletic Uniform must be Navy Shorts and Gray Shirt Must have the LSOC Logo or be blank Sweats are the same, LSOC Athletics logo or be blank

Requirements for Participation (cont.) No Pass/No Play Students must maintain passing grades in ALL classes in order to be eligible to participate Students who fail multiple six weeks will not be allowed to remain in athletics Practice/Game Pick Up Students must be picked up at designated times from practices and games No Pass/No Play Coaches want to go home to see their own families, too. Habitual offenders can be removed from the team.

Practices are mandatory Student behavior will be monitored by coaches Must uphold Life School Character and Mission statement Multiple disciplinary issues could result in removal teams or athletics Defined in the handbook Final decision made by Athletic Coordinator Practice are mandatory Morning & off-site practice Student behavior is monitored Students that don’t meet the Character expectation at school will not be given the opportunity to let us down at another campus Life School has a tremendous reputation around the state. We will put it in jeopardy Discipline at during school is just as important as discipline during games & practices

Additional Information Asthma Doctor’s note and Life School Nurse form to keep inhalers on hand (extra for sport med kit) All health related issues or illnesses will need to be reported to the Athletic Trainer and coaches with a doctor’s note Only a doctor’s note will be accepted as an excuse not to participate in workouts Injury evaluation, Treatment, and rehabilitation can be provided by the Athletic Trainer on campus. REPORT ASAP. Asthma Inhalers need to be with the student at all time Must have a note from the Dr. and form filled out in the nurse’s office allowing permission All health issues must be reported with a Dr. note It gives us insight into treatment and/or special protocols to follow Protects the athlete from themselves (tells the coach what can & cannot be done) Injuries vs Soreness See the trainer first See a doctor for major symptons Ask the doctor what you CAN do Hand injuries can still run/foot injuries can still lift We provide focused treatment of all injuries which can diminish down time

Paperwork and Physical The best way to assure that your child is able to participate in athletics is to make sure ALL paperwork and the athletic physical are complete. Get the physical completed in the summer before school. Bring your physical to Open House in August.
