2017 HUD CoC Program Competition Application part 1 Agencies wishing to renew or apply for funding in the 2017 HUD CoC Program Competition must: Review the North Central Florida Alliance CoC Ranking and Selection Process as posted at: http://www.ncfalliance.org/coc-ranks-and-exhibit-1/ Review the HUD CoC Notice of Funding Availability: https://www.hudexchange.info/resource/5419/fy-2017-coc-program-nofa/

2017 HUD CoC Program Competition Application part 2 Assure agency meets eligibility criteria: Is a non-profit 501c3 or meets other eligibility requirements established by 24 CFR 578.15 Actively participates or agrees to participate in HMIS Submitted application through eSnaps; and all supplemental forms and attachments are received in ACCHH office by deadline (5:00 p.m. August 22nd) GRACE, 3055 NE 28th Drive, Community Services Center (Building 7, Teal trim) Documentation of match and leverage are included The project serves an eligible project type Project follows “Housing First” approach as documented in agency policies and procedures

2017 HUD CoC Program Competition Application part 3 Submit the following documents: Renewals Policy and procedures for policy documenting participation in Coordinated Entry, HMIS and following Housing First Completed application from eSnaps Annual Performance Report eLOCCS printout 2017 HIC/PIT submission

2017 HUD CoC Program Competition Application part 4 New projects Policy and procedures for policy documenting participation in Coordinated Entry, HMIS and following Housing First Completed application from eSnaps Org Chart HMIS reports indicating multiple funding streams Award letters 2017 HIC/PIT submission

We’ll provide to reviewers We will check list and advise reviewers We will print HMIS report; you need to provide your Policies and Procedures and a PDF of your application We’ll provide to reviewers Submit copy of APR and highlight these items
