SSI – Supplemental Security Income Social Security Administration: SS – Retirement Income after paying into SS SSDI – Disability Income for those who have paid in to SS SSI – Supplemental cash income for those who are disabled and have not worked enough to qualify for SSDI Household income is considered prior to the age of 18. After age 18, income is based on individual’s income. Information on household income and expenses will be requested. Medicaid / Kancare MCO – Managed Care Organization – 3 Insurance companies contracting with the state to provide medical insurance – Medicaid (United, Sunflower, Amerigroup) Required to apply for SSI if applying for Medicaid Presumed Eligible if Receiving SSI May still qualify even if the individual is unable to receive a cash supplement through SSI due to household income.

Home and Community Based Services- HCBS Provides assistance in order to support individuals so they will be able to remain in the community. The services received will vary depending on the waiver the individual qualifies for and the person’s individual needs. Provides oversight for persons in Kansas with disabilities. Providers include a variety of private not-for-profit and for-profit organizations as well as governmental entities. Guardianship Guardianship is a legal proceeding in which someone (usually a family member) asks the court to find that a person is unable to manage his or her affairs effectively because of a disability. A guardian steps in the shoes of the person with a disability and makes the decisions for them. Different types of guardianship are available based on the specific needs of the individual: Guardian of the Person / Guardian of the Property Families typically begin this process to obtain guardianship a 3-6 months prior to the child turning 18. *If the student has a case manager, the family may want to talk with them as there is sometimes assistance available to cover the cost of obtaining guardianship. The family may also want to consider developing a special needs trust in order to protect the students benefits.

Vocational Rehabilitation Services Kansas Rehabilitation Services is a state run program which helps people with disabilities achieve employment and links businesses with qualified and productive employees to meet their workforce needs. Rehabilitation Services partners with community-based organizations, contractors and service providers in the provision of vocational rehabilitation (VR) services.  Such services are intended to empower VR consumers to achieve their goals for competitive, integrated employment.

How Can IEP Case Managers Assist Families With Accessing Community Services Become knowledgeable about available supports and services. Utilize your building school social worker and/or vocational special needs coordinator as resources for families. Talk with families and students regarding transition and community services….start early! Encourage families and students to attend Transition Panels and Community Fairs. Work with the IEP Teams to accurately reflect student needs and services being provided in school Provide age appropriate transition assessments and address post secondary goals. Create meaningful graduation plans and carry them out with fidelity.
