Quality Profile Definition: “The Quality Profile is timely, comprehensive, reliable information that describes the Quality of Care provided in a way that drives and evaluates improvement”

Measurement for Improvement Team “Information is a source of learning. But unless it is organised, processed, and available to the right people in a format for decision making, it is a burden, not a benefit.” William Pollard (1828-1893)

What is the Quality Profile? The Quality Profile is timely, comprehensive, reliable information that describes the quality of care provided in a way that drives and evaluates improvement. It is a tool that primarily compares your service as it is now to your service in the past. It is timely. It is owned by the service. It is an honest appraisal of the quality of care provided. It is a tool that contains both Quantitative and Qualitative information. It is a tool that helps users to identify interventions that work.

National Quality Profile AIM: Develop a National Quality Profile for the HSE Leadership Team that presents information included in the Performance Report in a way that drives and evaluates improvement Data in the National Quality Profile are presented graphically in: run charts which display the trend over time Statistical Process Control (SPC) charts which contain control limits where possible. Members of the QID Management Team were involved in the initial stages of the project and wider engagement with key stakeholders across the HSE is now ongoing.

National Quality Profile example Delayed Discharges (monthly data)

National Quality Profile example Delayed Discharges (weekly data) The number of delayed discharges increased during 2016 from an average of 556 in Jan/Feb to an average of 609 since March. The number of delayed discharges is expected to vary between 567 and 652 each week by chance alone. The number of delayed discharges has fallen below the lower control limit in the last 2 weeks (1st and 8th Nov) which can be interpreted as a signal of improvement. Further data points will be required to provide a signal of sustained improvement. The number of delayed discharges remains significantly above the target of <500.

Primary Care Quality Profile AIM: Develop a National Primary Care Division Quality Profile for the Primary Care Quality and Safety Committee Working in partnership with the Lead for Quality and Safety and the with the Quality & Safety Committee in Primary Care division. Workshops held with Governance Team for the PCD quality and safety committee members Focus on outcome measures for the Quality and Safety Committee

Primary Care Quality Profile example Rate of Hospitalisations with a Principal Diagnosis of COPD per 100,000 Population, National Level, January 2014 – June 2016

Social Care Quality Profile AIM: Develop a National Social Care Division Quality Profile for the Social Care Management Team in order to drive and evaluate quality improvement and inform strategy by Q4 2016. Working in partnership with the Lead for Quality and Safety and his team in the Social Care division. Project sponsored by QID National Director and SCD National Director. The current Quality & Safety Report for the SCD Senior Management Team and the SCD Quality & Safety Committee is being transformed into a quality profile by applying measurement for imporvement principles to existing datasets.
