AHIP is an opportunity to take control of your health Accountable Health Improvement Plan (AHIP) AHIP is an opportunity to take control of your health This comprehensive program will be the key to holding you accountable to your health goals by making you aware of your health status, commit to making a change, and seeing the change through. Preventive health care is one of the major steps to your physical and mental wellness and can help you better understand your body. Completing your annual biometric screening and health assessment increases your chances of maintaining good health and early detection of potential health problems. Following the screening and health assessment, customized tools will be available to assist you with improving your health and keeping you on track towards your goals. Pay less for your medical benefits each year if you participate and complete the required activities. If you paid for this type of program yourself, it would cost you well over $500 for all of the tools and resources provided to you through AHIP.

Benefits of AHIP As a member firm of ACEC Life/Health Trust, we are currently included in a health insurance risk-pool that is healthier than the average industry. Firms in AHIP will be placed in a separate, even healthier risk-pool, with the potential for lower medical premiums while improving the health & wellbeing of our employees and families. Cost savings realized by the firm will enhance our competitive position and allow our firm to invest even further into our most valuable asset – our employees. The move to AHIP is the next step to advancing our wellness initiative and making our organization more proactive towards our health and wellbeing. In addition, we will provide new health improvement program tools to those in need.

We have engaged RedBrick Health to administer the Accountable Health Improvement Plan and share a steadfast commitment to privacy. ACEC Life/Health Trust, your employer, and your insurance carrier will not receive any individually identified personal health information collected during your participation in Designed Wellness without your written authorization or unless required by law. The RedBrick Health web portal is secure and HIPAA compliant to ensure that you are the only person who is able to access your personal information, which includes the results of your Personal Health Assessment and Biometric Screening. ACEC Life/Health Trust and your employer only receive aggregated participation information from the data collected. RedBrick notifies ACEC Life/Health Trust as to whether or not you have satisfied the AHIP program requirements to determine if you qualify to pay less for your medical benefits and receive incentives earned.

Eligibility Who is eligible to participate? All employees & spouses enrolled in ACEC Life/Health Trust medical plan Do I have to pay for the programs offered? All programs are available free of cost! Why should I participate? Employees & spouses who complete their activities will pay less for their medical benefits and are more likely to maintain or improve their health

Participation Requirements To pay less for your medical benefits, all employees/spouses enrolled in a Trust medical plan must : Complete the online personal health assessment. Complete a biometric screening. Screening results will be automatically sent to your AHIP account and direction will be given based on the following: If Metabolic Syndrome is diagnosed (3 or more risk factors out of normal range), a health improvement program must be completed. If Fasting Glucose exceeds 100, regardless of the results on the other 4 factors, then a health improvement program must be completed. If there is no MetS or elevated glucose after initial biometric screen, the participant will have no further requirements to maintain lower premium. Attest to participation in Designed Telehealth and Designed Advocate programs. Complete a Next Steps Consult call

Health Improvement Programs Based on results of your biometric screening, you may be required to complete a minimum of 8 weeks of one of the following health improvement programs:

In order to successfully complete the program, you must: Naturally Slim is a behavioral program that helps you change how you eat instead of what you eat. A online clinical weight loss program that is based on learning new skills about when & how to eat, not dieting. Each week you will have access to approximately one hour of video lessons. Additionally, you have a dashboard to track your weight loss progress as well as tips & tools to keep you motivated and allow you to communicate with coaches & other participants. In order to successfully complete the program, you must: Complete at least 8 of the 10 online video courses for Naturally Slim Foundations within their set weekly time frames. It will be your responsibility to ensure your participation is accurately recorded each week. Complete a survey after completing the Foundations portion of the program.

Customize lifestyle coaching in a 1:1 setting and group sessions An intensive lifestyle intervention experience for individuals who will benefit from clinically relevant weight loss. Participate in a minimum of 8 weeks of engaging, intensive, customized lifestyle support. Customize lifestyle coaching in a 1:1 setting and group sessions 24/7 access to the secure portal with additional tips and tools for healthier living: Support tools provided: Blender, scale, food scale, pedometer, portion plate, water bottle, measuring cups and spoons, session guide, nutrition guide, workout DVD and more. Online tracking tools

AHIP Metabolic Syndrome Scoring System The first step to better health is knowing your strengths and risks. Start with your health screening and the Designed Wellness health assessment, then complete a Next-Steps Consult to review your results and get guidance on your health status. $200 Designed Wellness Incentive In addition to paying less for your medical benefits, each participant is also given the opportunity to earn a $200 incentive. To earn the $200 incentive, a participant must earn the full amount by completing any combination of the below activities. Incentives are “all or nothing”. The entire $200 must be earned with no partial incentive payout. Completion Deadlines Activities start on January 1, 2017 Step 1: Register on the web portal and schedule your biometric screening Step 2: Complete biometric screening by April 1, 2017 Step 3: Complete the remaining AHIP requirements by May 1, 2017 Health assessment, Next Steps consult call, and Designed Advocate and Designed Telehealth participation Step 4: Based on results of biometric screening, complete a minimum 8 weeks of a health improvement program by September 1, 2017 Naturally slim OR Real appeal The $200 Designed Wellness incentive plan is earned when $200 worth of activities are completed by December 31, 2017. Incentive plan payments are distributed in January 2018.
