LATENCY In case of unemployment and perfect suspension of work, you will enjoy coverage in Simple Layer through Pacific EPS, and coverage in Complex Layer through EsSalud. Important: The services offered by Pacífico EPS will be made only to Credit in the clinics specified in your health plan: Network 1 and Provinces A.

Law of Modernization of Social Security in Health Latencia Article 37.- In case of unemployment or perfect suspension of work that generates the loss of the right to cover, regular members who have a minimum of five months of contribution, consecutive or non-consecutive, during the 3 years preceding the cessation or suspension And their right-holders, have the right to the medical benefits provided in Articles 11 and 12 of this regulation, at the rate of two months of latency for every five months of contribution. The periods of latency that are generated can not exceed twelve consecutive months, counted from the date of cessation or loss of coverage caused by the perfect suspension of work. The contribution periods that have been effectively computed to grant latency periods will not be considered for the computation of the qualification periods of the next latency periods that are generated. The benefits recognized by the special right of unemployment coverage or perfect suspension of work, will be provided according to the conditions established in the previous paragraphs, as follows:

WHO IS GIVEN THE BENEFIT OF LATENCY? The Latency benefit is granted only to Pacific EPS affiliates and their dependents.

HOW AND WHERE ACTIVE MY LATENCY PLAN? 1. Go to your company's Human Resources area Request the following documents: • Last pay slip •Job certificate • Settlement of social benefits

HOW AND WHERE ACTIVE MY LATENCY PLAN? 2. Schedule the Unemployment Coverage Letter in Health: Go to the center of Es Salud to which you are attached, if you do not know it you can consult in www.essalud.gob.pe by entering the section "Insured". You must present the following documents in order to obtain the letter of unemployment coverage: • Last payment slip in original and copy. • Original and copy work certificate. • Liquidation of Social Benefits in original and copy (it is important to present the liquidation since sometimes in the system of Es Salud does not appear the declared work stoppage by the company). • Form 1022 signed by the worker in original and copy. This form is delivered in Es Salud or you can download it directly at www.essalud.gob.pe in the section "Insured - Trámites y Gestiones". • DNI in original and copy. Es Salud will deliver the Unemployment Coverage Letter. After receiving, you will have to approach the accreditation of the Health Clinic that corresponds to you presented your Identity Document and a copy of the Letter of Unemployment Coverage for the update of the coverage of care by complex layer in the system of Es Salud.

HOW AND WHERE ACTIVE MY LATENCY PLAN? 3. Calls for Latency Coverage in the Pacific: Once obtained the Letter of Unemployment Coverage, you can process your Latency in Pacific EPS via email or by going to Customer Service Platform in the Torre de Pacifico Seguros, Av. Juan de Arona 830 - San Isidro.

HOW TO PROCESS YOUR LATENCY VIA EMAIL? Once obtained the Letter of Unemployment Coverage you must send the following scanned documents to the mailbox trama_emision@pacifico.com.pe: • Letter of Unemployment Coverage issued by EsSalud. • Identity document of the holder (DNI or foreigner's card). • Settlement of Social Benefits. The subject of your email should read: Latency Procedure, full name of the holder, ID and company name. In a period of no more than 48 hours, we will send to the same email the confirmation that your Latency Plan is active and we will attach the said Plan in PDF.

HOW TO PROCESS YOUR LATENCY IN QUIET PACIFIC OFFICES? Go to the Platform of Customer Service in the Torre de Pacifico Seguros, Av. Juan de Arona 830 - San Isidro, with the following documents: Original and photocopy of the Letter of Unemployment Coverage issued by Es Salud. Copy of the Identity Document of the holder (DNI or foreigner's card). Photocopy of the Settlement of Social Benefits. After presenting the documents, we will give you a list with the authorized clinics. In order to assist you in the clinics, you will have to present your ID. NOTES: During the Latency Period the refund system does not apply in any case. The Latency Period ceases to be effective when you start working on a payroll at the second month of entering the new company or if you have exceeded the latency period. LATENCY COVERAGE PERIOD The Latency coverage period varies according to the number of contributions you have made in the EPS. To enjoy this benefit you must have contributed at least five (5) months during the last three (3) years prior to termination. For every five (5) months of contribution, you will have one (1) month of Latency coverage. The Latency period will apply from the date that you no longer have coverage in the regular EPS health plan.

LATENCY COVERAGE The member is entitled to the benefits referred to, even in the event that his employer has failed to pay the contributions or remuneration corresponding to the periods of contribution computable to access the special right to unemployment coverage or perfect suspension of Without prejudice to the powers of ESSALUD and the EPS, when applicable, to determine and collect the obligations that may be owed by the employers for such periods.
