Go to Section 1 KFSH RN Edu Questionnaire Select “Add” If you are putting information for the first time

Section 1 KFSH RN Edu Questionnaire Area 3 If you did not do any Non Nursing education, you will choose “0-Not Applicable” and leave the other questions in this area blank

Section 1 KFSH RN Edu Questionnaire Area 3 Please do not put your high school or “A” or “O” levels diploma/certificate under Non Nursing education . If you do not have information to put in a field then leave it blank. Do not put in “NA” or “Not Applicable” unless you are indicated to do so

If you do not need to fill out the sections on Speciality Cert or Prof Organization Then go to the top of the page Make sure you click on “Next”

The next page will give you a summary of the data you inputted Check the data to see if it is correct If the data is correct, click on “Submit” If the data is not correct, click on “Back”

Go to Section 1 KFSH RN Edu Questionnaire Select “Update” If you are editing information

Section 2 KFSH RN Speciality Cert An example of a speciality certification is CPHON®(Certified Pediatric Hematology Oncology Nurse). The credentialing body is the Oncology Nursing Certification Corporation (ONCC). Renewal of ONCC certification is due every four years. For all speciality certifications you have to take an exam and pass it. Once you pass the exam you will get a credential number. Candidates must maintain their certification and apply for certification renewal. If you do not recertify by the expiration date then your certification will be revoked.

Section 2 KFSH RN Speciality Cert Another speciality certification is Certified Professional in Healthcare Quality (CPHQ). The National Association for Healthcare Quality (NAHQ) is the credentialing body. The recertification cycle is 2 years in length for the CPHQ.

Section 2 KFSH RN Speciality Cert This section is NOT for: Saudi Commission for Health Specialties license Life Support Training Courses-BLS, ACLS, PALS or NRP Basic degrees or diplomas in Nursing- B.Sc.N, Post basic degrees, diplomas, certificates in Nursing Modules from your degree or diploma Nursing licensure board exams Course work for IV insertion, epidural, PCA, CRRT, Wound or Ostomy care NCLEX, CGFN, TOEFL Professional Registration Commission or Board of Nursing information

Section 2 KFSH RN Speciality Cert The components of a speciality certification is that There is a credentialing body involved It needs to be renewed on a regular basis. A credential number is provided by the credentialing body There is an expiration date associated with your speciality certification. It is not granted by an academic institution

Only go to Section 2 KFSH RN Speciality Cert if you have a Speciality Certification. If you do not have a speciality certification do not select “Add”

Section 2 KFSH RN Speciality Cert As you can see all the fields have an asterisks “*” in front of them. This means they are mandatory fields. Make sure you have the information that is required for each mandatory field

Section 2 KFSH RN Speciality Cert Once you have completed the form Review all the information to see if it is correct and Then go to the bottom on the page and click on “Apply”
