Nottinghamshire Carers Hub Support Services for Carers Presented by: Christine Alexander CEO

A support service for carers… Delivered by Carers Trust East Midlands (previously Crossroads Care) Funded by NCC and the six CCG’s Service started August 2015 and is until March 2018 90,698 carers in Nottinghamshire County (2011 census) Universal service - Support to all carers across Nottinghamshire County – BGR, Mansfield & Ashfield, Newark & Sherwood, Bassetlaw (excl. Nottm City) ‘Cared for’ needs to reside in Nottinghamshire County

Overview of support model Centralised helpdesk with community presence The Nottinghamshire Carers Hub can offer: Information, advice & guidance, signposting Carer Groups and ‘drop in’ sessions (14 in operation) Short break - holiday homes (based in Suffolk) Health and Well Being facilities via free gym memberships Access to grants (up to £500 per carer) Carers Smart discount card Carers respite breaks (via volunteers) Free training Carer consultation and engagement Support to carers to complete an online assessment Crisis planning
