Transportation Requests cmERDC Student Plans
Initiate Transportation Request (Case Manager)
Select student to create request Click on transportation link on left student menu bar Click create new transportation request Student must have an active SpED plan, 504 plan or Health Plan
Click on request special transportation Enter date of request (not the date to start) Click submit
Fill out transportation request Select associated plan from drop down list Enter start date, stop date is optional (ie. For a limited time like a student with a broken leg needing transportation)
Student address types are determined by SpED staff and setup by system administrator (ie. Home, daycare, etc) Select alternate address if applicable (not in So. Wash) This information is not part of the request, only used to fill in information below
Select school/program student if different than default Contact name, phone number and grade is required.
Select pick up address type Select days for pick up Contact information should be filled in Address drop down information is pulled from the top of the form and used to fill in the following fields
Select drop off address type Select days for drop off Contact information should be filled in
Select all Needs that apply Type any notes that might be useful for transportation department Click submit when done
Print transportation request
Check completeness Finalize request if complete
Click on print PDF icon
Verify Transportation Request (Case Manager)
Verification Process Export from Edulog Import to Student Plans Print verification report to give to case managers Case managers will review report, modify or fill in missing info. Select change status to not needed if necessary
Search for Transportation Request (transportation staff)
Click My Transportation Cases
Select student
Click ‘not set’ to input route information
Fill in route information
Click My Transportation Cases
Search by case manager
Click create to input a new request
Run Transportation Batch Report
