North West Resettlement Hub February 2017

What is a resettlement hub? Resettlement is when you help People to move to a different place A hub is a way of working that brings together people and resources

Resettlement Hub: Priorities: People in hospital longer than 4 years Need extra help to move back into the community May have restrictions and need more support to stay safe May need discharge plans arranged over a longer time

People with complex needs  People with complex needs – All people who are in hospital and it is difficult to find ways to help them leave hospital Hub can provide additional help with planning Examples: Housing – somewhere to live Working with Ministry of Justice Person centred care packages Support to make choices over where to live Work with commissioner to problem solve and work together across localities

People who should have been discharged from hospital These people have not been discharged from hospital on the planned date A delayed discharge is when a patient, is clinically ready for discharge, cannot leave hospital because the other necessary care, support or accommodation for them is not readily accessible and/or funding is not available.

Young people in transition, 16+ To help children aged over 16 years who are in hospital to move out of hospital To help plan for when children leave school residential placements To stop children at 18 years old moving into admissions to adult inpatient services

How will the hub work Work with experts by experience Lead on projects Check progress of discharge plans Help the persons case manage plan and solve problems Work with people, their families and professionals Help make systems work better together Help everyone involved work together Get help from a range of professionals from other areas Tell people what is happening- reports Tell NHSE when there are problems and risks which need help to solve Share learning Help people do things differently

How will we know the hub is successful ? After 3 months We will ask people We will check people are leaving hospital After 6 months and 9 months We will share learning At 12 months There will be a formal review . The hubs will end 31st March 2018
