Field Education Application Please fill out the field education application, print one copy (part A & B) and hand to Russo and email (part A & B), to Russo: Student Field Education Forms Due: April 7th by 11am. Please make sure to list any legal issues/problems, if you do not and it comes out through a background check you could be dismissed from field education and/or from the program Also give me a one page summary of why you are the best person for that placement—(your first choice) Each of you will be contacted in June through email once the placements have been decided, make sure to check your emails Once you receive my email you are to contact your agency and have a formal interview, training, background check, etc

NASW/Malpractice All students have to register or renew their NASW membership—approx. Fee: $40.00 All students must have malpractice insurance through the NASW assurance program or other source before they are able to work with clients—make sure you get $1,000,000--$3,000,000 fee varies: $10-$35.00 If you do not obtain Malpractice insurance you will not be able to start your field education experience If you have difficulty obtaining malpractice insurance through NASW please contact me there are other options

Expectations in Field Education You will be expected to complete 500 field hours: 250 hours in the fall and 250 in the spring Fall: 235 hours in the field, 15 hours in class Spring: 235 hours in the field, 15 hours in class Expected to come to every field education seminar class, just like you will be expected to go to your agency to complete your hours—any absences will have to be made up whether for seminar or agency Expected to participate in a minimum of 1 hour of supervision per week, explain. Expected to dress professionally and act in a professional and ethical manner, explain.

Expectations in Field Education Student is expected to treat their field liaison, field instructor, staff, and (most important) your clients with respect. Student will be evaluated twice each semester through midterm and final evaluation, explain. Student will keep track of hours at field education site with a time sheet you fill out weekly, make copies of everything. Student will fill out a weekly supervision log. Student will write a summary or process recording each week, obtain journal articles, and discuss ethical dilemmas. The field liaison’s are the first point of contact if there are any problems, concerns, etc for both student and agency.

Expectations in Field Education Situations that may arise: What happens if you don’t get chosen for a field agency during field fair? What happens if you get terminated from your field agency? Field Instructor or you oversteps boundaries? Supervision?

Field Fair Info Expectations Dress Professional Performance Minimum of 5 field instructors—you have to meet with at least 5 field instructors, once you hand in your top 5 agencies you may leave, we always suggest speaking to more agencies than 5
