Electronic Device Agreement Signed in the blood of employee ... Queens Library Staff Electronic Device Agreement I. Overview Queens Library is committed to providing an appropriate electronic device for communication and research purposes, including but not limited to eReaders, non-Blackberry cell phones, MP3 players, video pc tablets, video cameras, microphones and game platforms. Use of these electronic devices should be within the standards of good judgment and common sense, in compliance with the Library’s published policies on acceptable use and as required through the terms and conditions of applicable software license agreements. II. Procedure If you want to purchase an electronic device you must obtain the written approval of the CEO/Library Director. Once approval is granted, if needed staff can contact ITS Director for purchasing advice and assistance. III. Responsibility & Ownership It is the staff member’s responsibility to take appropriate precautions to prevent damage or loss/theft of the purchased electronic device. The staff member may be responsible for certain costs to repair or replace the electronic device if the damage or loss is due to negligence or intentional misconduct. o Ownership of the electronic device will reside with the Library and must be returned when employment ends, or the Library requests its return. o ITS support of Library owned electronic devices will be provided while on library premises. V. Theft If the electronic device is lost or stolen on or off the premises it must be reported to LSM, local police department and ITS immediately. The police report should include the serial number for the lost device. A copy of the police report must be sent to LSM and ITS within 48 hours of the discovery of the loss. VI. Upgrades and Troubleshooting When an electronic device requires hardware upgrade (e.g., memory, peripheral, or hard disk), software installation, and firmware update or has problems that cannot be resolved over the telephone, the device will need to be brought to ITS for hardware service, software installation or problem diagnosis. VII. Software Licensing Whenever possible, the electronic device will be configured with a standard suite of programs based upon the Library’s software standards; this may vary depending of device purchased by user. It is also possible that the library, based upon your professional needs or the requirements of the device, will provide other applications. Periodically, the Library changes software agreements; when software agreements change, you must bring your device in for an update. Only Library licensed software may be installed on the electronic devices. VIII. Offsite Internet Access ITS will not provide Internet access to you from off the library premises however will assist in with configuring your device to work with your ISP. IX. Backup You are responsible for maintaining an appropriate backup of your electronic device to prevent from data loss. If VPN access is provided then a network share can be used. X. Virus, Hacking, and Security Protection Users must take responsibility for ensuring that security updates take place on electronic devices in their care. To ensure that virus protection and other security patches are current, on your device, you are required to make an appointment with ITS once per quarter for maintenance. ITS will make every effort to return your electronic device the same day as the appointment. * A new agreement will need to be signed annually or when changes are made to the form.
