Overall good and stable performance Performance Summary Overall good and stable performance Excellent performance in Radiation Protection Improvement in rating of Performance Improvement Satisfactory and stable performance in Operations Indication of performance gaps in Chemistry and Learning Services Decision to not re-license plant has not adversely affected safety culture or performance + Ongoing monitoring area for Quality Verification Roles and responsibilities are not clearly understood within the emergency preparedness organization. Communications are hampered within the team due to the unclear roles and responsibilities. Written guidance for drill and exercise preparation relies heavily on past experience and tribal knowledge. 2

Top Issues - May 1, 2016 – Present Confined Spaces Identified administrative non-compliances with confined space regulations and enhancements to confined space entry activities during 2R19 Follow-up in preparation for October tunnel cleaning identified minimal action taken Issue elevated to Station Director Action taken prior to start of tunnel cleaning 3

Top Issues - May 1, 2016 – Present Operations Verification Practices Multiple examples of poor use of human performance tools when clearing or restoring plant equipment Complacency in implementation of standards when risk and consequence are judged to be low Although risk/consequences low, behavior could challenge higher risk activities Action plan developed to address issue - actions in progress Security Audit Findings Failure to properly check radio functions Inconsistent use of security event logging software 4

Top Issues - May 1, 2016 – Present Emergency Preparedness Change management issues have led to unclear roles and responsibilities, potentially impacting the effectiveness of the Emergency Planning team Actions taken to define roles and responsibilities and to assure that emergency exercise training and preparedness are adequate Emergency Planning audit concluded that program is effectively implemented Material Handling Material handling practices during 2R19 resulted in RCP rotor slipping from pallet jack Roles and responsibilities are not clearly understood within the emergency preparedness organization. Communications are hampered within the team due to the unclear roles and responsibilities. Written guidance for drill and exercise preparation relies heavily on past experience and tribal knowledge. Material Handling issue led to: Required rotor to be sent to Westinghouse for inspection Potential impact to 2R19 outage schedule 5

May 1, 2016 – August 30, 2016 Quality Performance Assessment Report Conducted 3 audits, 16 assessments, and 140 observations Internal Auditing Emergency Planning, Security and Training 4 findings, 37 deficiencies, 16 recommendations Assessments 2R19 refueling activities, radiation protection postings, emergency planning drill preparation, clearance process, fire protection program 1 finding, 15 deficiencies, 34 recommendations . 6

Wrap-up Overall plant performance remains on a positive trajectory, but need to ensure that standards issues being seen in some departments are corrected to assure a continued positive trajectory 12
