Progress So Far – Newport City Council Outcomes Project

The Brief Stage 1 -Designing the tools to develop an outcomes focused approach. An outcomes focused assessment and care plan An outcomes focused service delivery plan An outcomes focused contract Stage 2 – Developing a training strategy, programme and materials to train and inform key staff in relation to the outcomes focused approach. This would involve writing the training programme and developing the relevant materials for staff in Newport to train other staff in this approach. It would also involve a workshop focusing on ‘training the trainers’. Stage 3 – Finance and market management. The work would involve working with key managers to discuss and agree the way forward in relation to: Key financial considerations e.g. unit costs, focusing on financial models to assist an outcomes focus. Managing the market – working with providers on the outcomes focused approach and identifying key areas for commissioning / decommissioning.

Work Completed Senior management event – April 2008 Practitioner Workshop – June 2008 Practitioner Workshop – Nov 2008 Care Plan and Care Plan Review Developed Proposals for Outcome Focused Plans developed. Proposals for an Outcome Focused Contract developed.

Learning so far - Overall Outcomes work involves a different way of thinking. Outcomes work involves a culture shift for staff – need to recognise and acknowledge this. Introduces the concept of ‘reinventing social work’. Important to communicate this. Be clear about the priority outcomes that you want to change. Language is important. Current planning system needs to be improved and developed. Have we got the right performance information to inform the outcome focus e.g. performance linked to operational teams?

Elements of an Outcome Focused Plan To include: The outcomes to be achieved e.g. older people living independently. The population whose outcome you want to improve e.g. older people aged 75 plus. The indicators that you want to improve and target based on previous years figures e.g. how many people living independently now, care home admissions. The outcome targets

Elements of an Outcome Focused Plan The strategy for changing the outcome for example: Training staff Targeted re - ablement Service provision Service user feedback Service deficiency analysis Quality assurance Managing the provider market Making use of accurate performance data.

Learning so far – Planning for outcomes Don’t put too many issues in the outcomes statement Getting the right meaningful indicators isn’t always easy. We often know what we want to do but we need to put in why and how. What problem is being solved and how does it impact on the customer.

The Pilot Where are we going to pilot this? - Adult duty and assessment, and domiciliary teams How do we want duty and assessment teams to work differently? – assess and develop care plan focusing on outcomes.

How do we want domiciliary teams to work differently? Provide a real alternative to residential care Want people to be taken out to do jobs Not ‘to do for’, but ‘to do with’. Changing working conditions of staff e.g. staff on more flexible working contracts. Bank hours? Development of support workers in teams. 24/7 service More flexible delivery of service – time wise and task wise. Improved OA Monthly cost envelope

Next Steps from November 2008 Workshop Complete Care Support and Review Forms Choose and agree team to pilot work – do we want to pilot this on a patch system. Write the process we want care managers to follow. Train staff in new processes e.g. assessment, eligibility, care plan , review Train staff using case studies. Develop time frame for completing the work.

Supervision of monitoring of pilot within teams Agree the rules for the pilots Agree the parameters of the services that we will provide. Understand risk assessment issues Agree feedback loop from commissioning and service deficiency forms Invest to save case for additional staff during pilot Agree the provider who will pilot Identify skills and train them Look at how processes will change e.g. recording of hours
