DS-874 Vision must be 20/40 or better in each eye Peripheral vision must be at least 70 degrees in each eye Be careful of corrections, they must be initialed. DS-874 The doctor marked restrictions, but cleared the driver with no restrictions. Remember that you can’t blame the doctor for filing incorrect paperwork. It is the carrier’s responsibility. DS-874 Marked the person named above is physically or medically qualified. Should say with restrictions  

The examiner qualified this driver even though he marked two ten point items. What fails this test? DS-875 The regs say that the DS-875 and the DS-873 may not be done on the same day. Good catch. DS-875 Driver should be disqualified. Two ten point items marked Done on the same day as the defensive driving observation.
