Population Health Population health has been defined as "health outcomes of a group of individuals, including the distribution of such outcomes within the group". It is an approach to health that aims to improve the health of an entire human population.

Project Outline Multi-stakeholder Committee 10 regional and community groups Created the “Action Guide” August 2016 with funding from the Department of Health and Human Services For Anyone: Community leader Public Health professional Employer Healthcare provider Health plan administrator Policymaker Consumer

Action Guide Framework for multisector groups to work together to improve population health Brief/written in plain language to help communities achieve shared goals Designed for accessibility and implementation Organized by 10 key elements

10 Key Elements Collaborative self-assessment Leadership across the region and within the organization Audience-specific strategic communication A community health needs assessment and asset mapping process An organizational planning and priority setting process An agreed upon prioritized set of Health Improvement Activities Selection and use of measures and performance targets Joint reporting on progress toward achieving intended results Indications of scalability A plan for sustainability

Using the Action Guide Purpose Provides concise steps to design Population Health project Shares useful tools that are research tested. Discusses successes and lessons learned . Focus on community.

Leadership Leadership Across the Region - At a minimum, this should include representatives from public health, healthcare, and other key stakeholders who are strongly invested in the affected population. Reports to reference: Collective Impact Common Table Health Alliance

Each element is described: what it is why it is important Communication and Needs Assessment Elements 3 and 4 focus on creating audience specific communications, and assessing community needs. Each element is described: what it is why it is important and how it can be done with examples and references.

Organization and Priority Setting Common Criteria for Prioritizing Population Health Needs and Interventions

Elements 7, 8, and 9 Prioritized Set of Health Improvement Activities Priority topic or topics are identified through shared planning, assessment, and decision making. Measures and Performance Targets Public health measures exist through Institute of Medicine, 100 million healthier lives etc… ESRD measures will be more specific to population but can include important health measures for all ie smoking cessation Joint Reporting on progress Transparent, all stakeholders, timely, and easily accessible

Scalability Scalability – The ability to expand the initiative to other regions or groups. Assessed with project initiation, can expand with new discoveries through initiative Example – CMS State Innovation Models Initiative fosters testing and development of state-based models for improving health system performance through multi-payer payment reform and other system changes.

Sustainability Sustainability – The ability to stay productive over time. The ability to motivate structural changes can increase the likelihood that you will sustain the gains you make Georgia Health Policy Center Sustainability Framework - Identifies components that contribute to organizational and programmatic sustainability.
