Coalition of Western MA Hospitals/Insurer Partnership of 10 non-profit hospitals/health plan Initiated in 2012 to share resources and work in partnership to understand and respond collaboratively to regional community needs Completed 2013 Community Health Needs Assessment Identified mental health services for opioid addiction as a critical need Meet regularly to discuss collaborative opportunities Completed 2016 Community Health Needs Assessment

Major Findings from Focus Groups and Key Informant Interviews Our most vulnerable populations in the region include: Older adults Children and youth Latinos and Blacks Low-income & those living in poverty; in western Mass, there are unique poverty issues in rural and urban areas LGBTQ Veterans Disabled Refugees People with Mental Health and Substance Use Conditions

Major Findings from Focus Groups and Key Informant Interviews For elders, social isolation is detrimental to physical and emotional health Cultural is much broader than just race/ethnicity; it includes stigmatized groups such as LGBTQ and persons coping with mental health and substance use disorders Veterans: Concerns about seeking help for behavioral health; issues of confidentiality, stigma, seen as a weakness; concerns about PTSD and triggers in veterans with dementia/Alzheimer’s Need more interpreters, translation of materials, bilingual providers, and health education tools that are readable and accessible

Major Findings from Focus Groups and Key Informant Interviews Need more health care provider knowledge about LGBTQ health needs and how to provide care in culturally competent ways Trauma, inter-generational substance use, violence, and mental health issues are closely tied together and result in significant need for mental health and substance use disorder treatment services The overlay of poverty, poor housing, lack of 'living wage' employment and other social determinants of health contribute to poor health The opioid abuse crisis has shed more light on underlying fragmented systems of care and payment mechanisms
