Photography Competition 2016 ‘The Future of General Practice’ Name of entrant: Signature: Date: Terms & Conditions: Each entrant can submit up to 3 photographs with captions. Entrants must be GP trainees in Dorset or GPs working in Dorset. Photographs must be taken by the entrant and not copyright property of another person. Any identifiable person must have given their consent for the photo to be taken and entered in this competition (aware that T&Cs apply). A photograph can be enhanced or edited in anyway the person taking it wishes. The files should be jpeg, jpg, png or gif files. Winning entries will be announced by 1 August 2016. The GP Education Team (Wessex) will have the right, with no further permission or payment, to publish all or part of any entry, in publicity, research or educational material.
