Connected Communities: draft strategy & outline plan Draft Strategy - what we’re trying to solve - how are we going to deal with the challenge - outline plan of what we’re going to do

what we’re trying to solve This graph from our JSNA shows the scope of the challenge we face in reducing social isolation. what we’re trying to solve

what we’re trying to solve

what we’re trying to solve

what we’re trying to solve with an emphasis on Remote /Self Care and Decision support

what we’re trying to solve PPG survey headlines 65.7% of the respondents have access to the internet, but only 35.0% have registered to use this method to contact the surgery 4.4% have used the free iphone App to order repeat prescriptions 2.2% have used App to make appointments Individual patients made comments relating to the poor broadband and mobile phone signal in their area, to the fact that they preferred personal contact to using a computer and of being unable to use the internet to have their repeat prescription delivered to or by Boots. DIABETES SUPPORT GROUP The Sleights and Sandsend Diabetic Support Group has now been running for nine months and those attending our meetings have seen speakers talk on varied topics from diet advice to foot care. The feedback has been positive and we are hoping to attract more people by inviting diabetics from Egton, Danby and Staithes to join the group. We are now meeting on the first Thursday afternoon of the month at Sleights Village Hall, from 3.00 to 4.00 pm.

how are we going to deal with the challenge Engage Community - Use champions (possibly PPG) to come up with ideas and problems on Health and Wellbeing and Social Inclusion where technology has a role to play, we’re going to have to listen carefully and support their needs at the same time Ideas - Take ideas and work through creatively with technical advocates using insight Implementation - Identify and develop the technical requirements and implement solutions in a mixture of delivery methods eg co- production Learn - Constantly learn and evaluate - Measure improvement - Measure risk/benefit Scale - Scope out which ideas can be implemented County wide 8 members of the PPG with 14 in the virtual PPG

Start Up Phase Insight & Vision – Get the vision set within the council backed by data Initial Stakeholder Engagement – Community Engagement process with the Key stakeholders and share our thoughts. By communicating, consulting and listening to these key members we can gain further insight and understand the needs of the Community Definition Phase Idea Generation – Following on from the Initial Stakeholder Engagement talks, they come up with their ideas for their Community of Sleights Technical Requirements – We start to build a picture of what we need to meet technical needs

Delivery Phase Technical Implementation – We build up a picture of what technical solutions we can build and develop around the specification and scope agreed and proceed to test, install and test again. We use a mix of delivery models eg co-production Scale-up Review Sleights project. What technologies worked. What’s the best way to engage. Funding model. Write strategy for County
