Section 125 – Pretax option Cafeteria Plan This program gives you the ability to have your insurance premiums deducted from your pay check on a pretax basis. You can lower your taxable income. Pretend you make $1,000 on your paycheck and your insurance deduction is $100. You pay tax on $900 instead of $1,000. This option is available with your medical, dental, vision and some of the other selected insurance options.

Section 125 – Pretax option To participate in this pretax option your coverage is “locked in” and you can make changes only during yearly open enrollment or due to a life event such as: Marriage Divorce Birth / Adoption Death Change in Coverage Loss of Coverage

Blue Cross Blue Shield (bcbs) hIgh Deductible Health Plan (hdhp) BCBS – HDHP, how it works. S Network BCBS. You must stay in network. Team member (tm) pays full – discounted price of office visits, prescriptions, procedures, etc. Some preventive drugs have a copay. A list can be found on the Company website. List is subject to change. BCBS provides discounts on in network services. No co-pays.

Bcbs Hdhp ATCO helps lower your medical expenses. Health Savings Accounts (HSA) HSA helps with upfront expenses. Health Reimbursement Arrangement (HRA) HRA helps on the back end of your out of pocket expenses.

Health Savings Account (HSA) Doctor Visits and Prescriptions are counted against the your deductible. Use HSA dollars to pay for these deductible expenses with pretax dollars. Your HSA Account may be used for other qualifying out of Pocket Expenses, i.e. Dental and Vision. If the TM contributes to their own HSA the Company will also make a contribution. To receive ATCo contributions, the TM must contribute per payroll: Family $50 Single $25

HSA contributions will lower your Bcbs - Hdhp Family Single Deductible $12,000 $6,000 ATCO HRA -$6,000 -$3,000 Max Out of Pocket $6,000 $3,000 -tm hsa -tm hsa -ATCo hsa -ATCo hsa HSA contributions will lower your out of pocket expense

Biweekly Premium Family $126.41 Single $49.06 BCBS - HDHP Family Team Member ATCO HRA BCBS Coverage $0.00 - $6,000 $6,000 - $12,000 Over $12,000 First member of your family will have a $2,700 out of pocket expense. The rest of your family members will share another $2,700 expense. Individual Team Member ATCO HRA BCBS Coverage $0.00 - $3,000 $3,000 - $6,000 Over $6,000 You pay 40 % of the premium. Biweekly Premium Family $126.41 Single $49.06 ATCo pays 60% of premium.

Delta dental Network Delta Dental PPO Annual Deductible $50 (3 per family) Does not apply to Preventive Procedures Annual Maximum $1,000 per covered member Orthodontic Services $1,000 lifetime maximum Preventive procedures are covered at 100%. You pay nothing. Please note, schedule your routine check ups 6 months apart. Basic procedures covered at 80%. You pay 20%. You pay 40 % of the premium. Biweekly Premium Family $13.15 Single $4.78 ATCo pays 60% of premium.

Davis / Guardian Vision Network Davis Vision Exam Benefit $10 copay Frames $135 allowance. You pay any amount over $135. Lenses $25 copay. 1 pair of lenses every 12 months. Contacts $135 allowance (in lieu of glasses). You pay any amount over $135. Additional fees may apply to contact fittings. Biweekly Premium Family $8.51 Single $3.84

Guardian Life insurance Covers the TM for 4 times annual earning and 8 times annual earnings if death is due to an accident. Benefit is 100% ATCo paid. You pay nothing.

Unum short term and long Disability If you become ill or are injured off the job and are absent for over 14 calendar days, you will begin receiving STD benefit. This pays you 60% of your average wage. Benefit is 100% ATCo paid. You pay nothing. If you are absent for 6 months the LTD benefit begins to pay you. This pays you 40% of your average wage. Because there is a decrease in the benefit after 6 months, you are eligible to “Buy Up” the LTD cover so that the LTD benefit equals 60% of your average wage. This “Buy Up” is paid by you and the premium varies depending on your income.

These policies pay you, NOT the doctors. Accident Policy Includes Accidental Death Benefit. Includes Wellness Benefit of $60 dollars. Biweekly premium - Single $12.42 - Family $23.22. Cancer Policy Includes Wellness Benefit of $70 or $75 dollars. Biweekly premium - Single $18.18 - Family $32.28.

Important points to remember Always stay in network when using any insurance. Doctor’s offices, prescriptions, lab work, etc. Always show your bcbs card. Call the insurance companies if you have questions about coverage or invoices. Keep your EOB’s (explanation of benefits). Schedule wellness exams / checkups at the appropriate times to prevent additional charges.

Ajax Turner Company, Inc. Pays for 100% of your life insurance. This provides a benefit for 4 or 8 times your annual salary. Pays for 100% of the Short Term Dis. (coverage at 60%) and 100% of Long term Dis. (coverage at 40%). Pays for 60% of your medical and Dental Premiums. Provides contributions to the team member’s HSA. Provides HRA funds to team members who reach their out of pocket maximum.
