SPM Project Populate a Scorecard from a SPQ Make sure you are on the Documents Tab To start the process you will need to create a new Survey. Click on Actions, and then click Survey 3

SPM Project Populate a Scorecard from a SPQ Give the survey a unique title. Use the naming convention “Supplier name, Commodity or Agreement Title, mm-dd-yyyy Survey” Provide a description. In this case we are identifying that this Survey is to update the Supplier Performance Questioner Choose the Master Survey Click Create 4

SPM Project Populate a Scorecard from a SPQ Add the Participants. Most likely this will be the supplier. 6

SPM Project Populate a Scorecard from a SPQ Click the checkbox in the header row to select all of the line items Click Delete to remove ALL of the content. 7

SPM Project Populate a Scorecard from a SPQ Verify that there is no content in your survey Click Add, and then click Content From Library 8

SPM Project Populate a Scorecard from a SPQ Expand the Supplier Knowledge Area Click the radio button to the left of Supplier Profile Questionnaire Click Select 9

SPM Project Populate a Scorecard from a SPQ Check both boxes to ensure the values are copied both from the Survey to the SPQ and back and that any visibility conditions are brought over. Make sure that you select all of the SPQ content Click Copy 10

SPM Project Populate a Scorecard from a SPQ I have scheduled the survey to be sent out on a future date, so the button reads Schedule versus Publish 12

SPM Project Populate a Scorecard from a SPQ The Recipient(s) will receive an email with a link to the SPQ Survey 13

SPM Project Populate a Scorecard from a SPQ The Recipient(s) will respond to the first question. Once the recipient agrees additional questions will appear 14

SPM Project Populate a Scorecard from a SPQ The Recipient(s) will complete the rest of the questions, and then click Submit Entire Response. 15
